Hi Joy,
Yes she is Obese, but not so obese in the face that the creases rub
together.
The area around her nose and down the smile crease to her chin is reddened
and scaly, it wraps around the chin in a large round spot then goes under
the chin in that small crease as well, it follows the lines on both sides
of her face and nose.
her ears are also affected, scaling and peeling behind and in the creases of
the inside too. Where ever there is a crease, there is that going on.
Her underarms and groin are red but since it is moist there, the peeling
shows as beads of dead skin that can be rubbed off, then more produces.

Personally, I wouldn't go the topical route unless either the patient is
really desperate, and/or everything else has been tried, and failed. Even
then, I would be just as sparing as could be.

If there is fungus on the skin, there *is* fungus inside, which ought to be
addressed!!! Will the cream be suppressive? I dunno, but that would be our
(Hahnemann's) theory, and seems a possibility. But *for sure* you are
removing your best indicator as to whether or not cure is taking place. At
least, that's been my understanding, and comes straight from Organon...

I would be very hesitant to apply anything topically.
I have seen the truth of suppression again and again. So if it is not
really really desperate it is better never to do topicals.
Years back I saw a patient with some ?sciatica. I gave her Ruta
because she had pain in thighs descending a stairs. The next visit
she had itchy patches all over. On enquiry I found out that 3 years
back she had been bitten by a centipede and had similar lesions for
which she had applied a mixture of turmeric with sesame oil. This is
a 'grandma's remedy' for such conditions and considered very
innocuous. We waited and she recovered remarkably.
If this simple remedy could suppress and years later the lesion
return after the correct remedy then think about the various creams
etc. If the patient is willing after you explain things it is better
to give medicines instead of topicals. In fact there have been cases
where I have refused to take patients on after I explained to them
that some of their previous skin conditions might recur.
