Doctor's orders are followed. Care plan developed by the facility's director of care and the wife is not followed consistently.

Sometimes it helps to earn a reputation as a troublemaker. When my mom’s dementia worsened to the point where she had to go into a nursing home, I learned there’s a great difference between an institution with a specialized dementia unit and one without. She was in the latter for a few months, until we realized it was not working for her: too much stimulation, not enough training and programing geared to those with dementia. They’d simply lumped her in with the general rehab population.

We found another home with a locked dementia unit and well-trained staff — even the janitors were trained how to deal with dementia patients. They had lots of programs to keep the patients occupied and reduce restlessness. My mom discovered she loved singing! At any rate, she was happy — and so were we — until they moved the entire dementia unit one day to two different wings, without bothering to notify us.

This happens not to be legal in my state. I wrote the nursing home manager, his boss, and the state nursing home board to express our consternation. I knew the care attendants were unhappy about the move; I took their side. To the credit of the home, they listened to us and made changes, improving the training of staff in one wing and moving my mom to the other where the staff was more familiar with her and her needs.

A good nursing home should be responsive. They should also have quarterly meetings with family to discuss and modify care plans as needed. It helps to know the laws and requirements of your state as well as the nursing home (and corporate) chain of command. Access to the press can also be of inestimable value: no nursing home wants to be the center of a media expose.
