We have had a discussion on this issue which is enlightening as it can set one up on the right course of curing one's patient.
WHAT NEEDS TO BE CURED lies at the centre of the case. However it has been mentioned that changes in the patient should be singled for attention. Of course if a change has happened (and there is an aetiology with it) then fine and take this as a symptom, but you are lucky to have these changes presented to you - sometimes you are not.
But what about fears that someone has and they have had them from childhood? Some may have been inherited because the mother during pregnancy had those fears.
What about feelings of inadequacy and lack of confidence. The pt may have felt like this all their life.
The question that needs to be asked is whether this symptom is part of their 'stuckedness or not. If yes then attend to it.
I think this comes back to evaluating the symptoms of the patient and giving them weight. This is a skill in itself.
Those who were trained to repertorise by elimination understand this concept well because one of the pre-requisites is that one has to prioritise the order of the symptoms (as part of training we had to give a justification for each symptom and rubric).

One of the issues raised was the concept of someone being a clairvoyant. Do we take this as a symptom?
As Andrew clearly said, if something has happened and all of a sudden they become a clairvoyant, then absolutely YES.
[Need also to make sure that this is real and not a delusion.]
However, if their whole life is being centred around their ability of being a clairvoyant - what then?
I would say yes you would need to consider it because it is now a major part of their make up. It is not that their clairvoyance needs to be cured - but it is forming their presenting make-up - it sets them apart from someone else who does not have that ability.
Using my previous example of the detective searching for the murderer, it is another useful clue not to be missed.
