Dear All, as a compliment to what Soroush has offered regarding case taking
and presentation I would like to offer the following.

Presenting complaint:-

Location + the parts, time, modalities, extensions
Sensations + modalities
Causation
Concomitants

Grade symptoms as to their importance

The vitality of the client/patient

How clear is the picture

Is there a dominant miasm

Are there any weak organs

Are there any effects of past suppression or medication

Medical history

Are there any maintaining causes and stress factors

Sexual energy

Happiness factor - temper, irritability, crying, optimistic, pessimistic

Ability to change - repressed, closed, open,

Sense of ease - fears, phobias, worry, anxiety, careless

Sense of awareness

Self feelings - negative, positive etc.

Feelings towards others

Particulars regarding appetite (loves and hates) and thirst, weather and
temperatures, best and worse times of day and night.

Morbid feel of the case

Does the picture you have match the person

What is to be cured

Prognosis - it can be useful to have an idea of how the case will turn out
and have an understanding of any case management that there might be

Consider potency as well as prescription.
It isn't always possible to get a wonderfully rounded case history,
sometimes you have to go with the barest minimum information.

As far as the online cases projects is concerned it will be helpful to all
if cases can be presented along these lines if possible. It will reduce the
tendency to speculate and the need to ask what some consider to be
unnecessary questions.
