I know I have left this one a little late (the patient is in on Monday) but I wondered about your thoughts on what remedies to give for this surgery which SHE wishes to go through with next week!! This patient has very bad "urge incontinence" which has been controlled for the last 6 months on drugs (Detrusitol) very successfully. She would now like to come off the drugs hence opting for this surgery next week. She has done very well on Causticum with Kali Mur 6 followed by Carc which have sorted her presenting complaints of cough, sinus and deafness and then her longstanding mental and emotional problems. However she still wets herself when she coughs.
Although it would have been good if she had given me more than a couple of months to sort the urge and stress incontinence out, I have said that although it is her decision to go through this operation I will support her with remedies. A couple of weeks ago she had some Staph 30 as she mailed me about her anxieties about the pain and humiliation that the surgery will entail. These anxieties had eased according to the last e mail.
Some details about this TVT surgery from the leaflet:-
85-90% success rate and patient leaves hospital the day following surgery.
"It works by supporting the middle of the urethra and is carried out by local or spinal anaesthetic so the surgeon can adjust the position of the mesh at operation and achieve continence without emptying difficulties. The mesh consists of a permanent Prolene material surrounded by a plastic sheath with a strong needle at either end. A small cut is made in the vagina and the mesh is passed under the middle of the urethra to support it. Both needles emerge through the anterior abdominal wall so that the mesh is accurately positioned. A fine telescope is introduced into the bladder to be sure there is no bladder injury and then the position is adjusted and the needles are removed. The vaginal and abdominal wounds are closed and the patient returns to the ward. Complications include bleeding or bladder injury when the needles are passed, difficulty emptying the bladder, urgency and urinary infection. So far (3 year follow up!!!!!) there have been no significant (!!!!) long term side effects.
OK my thoughts on this are Ledum because of the needles, Bellis perenis - internal bruising, Calendula - to heal the wounds and Staphysagria- for the indignity.
Any one else have any thoughts on this? Now I am pretty sure the urging problem was to do with the M & E which I haven't got permission to discuss - unlike the surgery - and now this has been sorted with the Carc and EFT the urging may not have still been there had she stopped the medication but as she honestly said to me she couldn't tell me if this had improved as the allopathic medication sorted it completely. I found it difficult when I first took the case to confine myself to her presenting complaints which did not include this urging problem because it was under complete control. Whatever, it is now the surgery I need to help her over.
