I'm going to add my 2 cents' worth to whatever Rosemary says. Dr. Ramakrishnan's book is a template, a very useful one, but few of us have his experience in treating cancer, and the book by no means covers every scenario. Having been in your shoes (with a lung cancer patient), I felt more comfortable having Dr. Ramakrishnan take the patient's case after I'd resolved his presenting complaint (which was not the cancer), and I've consulted briefly with him once or twice since. That won't work for all patients (Dr. Ramakrishnan charges $500 for the initial intake), but you could arrange an e-mail consultation, which would cost about $150. For this to work, obviously, you have to take a very detailed case.

I personally did not feel confident enough to treat my first cancer patient without having a more experienced mentor hold my hand, so to speak. And if I had another case that wasn't clear, or that presented difficulties, I wouldn't hesitate to consult again with Dr. R.
