Hahnemann did notice that complex diseases can co-exist very very rarely in acute diseases but more commonly in chronic, and very very commonly acute over chronic.
His suggestions were in chronic diseases if Psora and syphillis are found togethre then give something for the psoric condition first then for the syphillitic and again for the psoric to root out the problem.
The same was suggested for Psora and sycosis and Psora syphilis and sycosis.
Now I do not want to get involved in any miasmatic theory. But put simple we have functional diseases(Psora), destructive diseases (Syphillis), and proliferative diseases (sycosis).
Even in these complex diseases if we do not find one remedy covering the whole picture we can simply Follow hahnemann's instructions of dealing with them one at a time.
Another thing we can do is to prescribe for the last appearing picture and keep on prescribing as the picture unfolds.

In an acute condition in a patient sufferring from a chronic disease we just considwer the latest symptoms (That means symptoms directly associated with the acute disease) we do not consider any of the chronic symptoms and prescribe only for the acute condition. After that we give the constitutional remedy.
