Free Homeopathic services: concerning trades or barters

I have a midwife friend who allows trades up to 20% of her fee, the work must be completed 2 months before the due date or cash is due instead. (The whole fee must be paid 2 months before the due date).
She has many Amish clients so has lovely furniture from them.
If they ask you for a trade or barter, you should set the conditions.
By the way, if people ask to set up a payment plan, I tell them they can send me $___ a month till they have paid for the initial consultation plus two follow ups. "Oh, I don't want to do that!" is the common response. But I'm not in the business of granting credit.
