Get his photos taken by a student of Grant Bentely and get the miasmatic remedy.
Also be sure he warms his feet several times a day with warm water. 15 minutes in a bucket of warm water. He needs to use a thermometer with the water. Put the bucket in a plastic bag. Wrap a soft rope of cloth to tie off the bag so the water stays warm longer. This will help him with the cold and circulation.

Gosh, A challenging case especially with blood flow compromised.
Calendula has had the best results for me in cases where circulation is poor. My own case of gangrene for example, came right with Calendula 200C, and I've seen many a diabetic ulcer also succumb to calendula.
The abuse you describe is enough to make one's blood boil.
How my gangrene got fixed: It involved the left back of thigh from knee to hip, and had resulted from a violin spider bite a year before. (Amputation was planned but then I "met" homeoapthy).
I took Calend 200C aqueous by mouth tid and had crushed Calend 200C sprinkled into the open wound sparingly, I think twice a day also, might have been three.
Amazingly, I do not even have a scar decades later, and I did develop alternate circulation, though not as perfecet as the other leg's circulation - I get edema a bit associated with my current illness (Cushings syndrome).
Calendula has a wonderful history of regranulating healthy tissue.
