Sciatica:
For the permanent cure of sciatica a constitutional remedy needs to be given. But at times the pain is so severe that a short acting remedy needs to be given first. Here is what I have found most useful
Am-m: In Ammonium Mur there is a feeling of contraction of muscles and tendons. Many times the patient expresses it as of a shortening of the muscles or sometimes shortening of the nerves. Sciatica where the pain is at its worst while sitting slightly better by walking and completely relieved by lying down is a combination that calls for Am-m.
Colocynth: Many times Bryonia is given where colocynth would do better work. The pains of Coloc are aggravated by motion, especially by continued motion. They are relieved by lying on the painful side, by warmth and by flexing the legs over the abdomen; any side may be affected but more often it is the right side that is affected. Like Gnaph, Coloc may have pain with numbness.
Gnaphalium: The pains of Gnaph. Are accompanied with numbness (Coloc, R.T.) or more specifically they alternate with numbness. The sciatica is worse lying and better by sitting.
Kali-iod: Many times Rhus tux is given where Kali-iod is the indicated remedy. The pains of Kali-iod are better by walking and by motion, there are aggravated by lying down, especially by lying down on the painful side. Night is not a good time for Kali-iod and all their complaints increase in the evening and at night. But if the patient gets out of the bed and walks in the open air he feels better. The pain usually goes all the way down from hip to heel. Remember Rhus-tox patient are sensitive to cold, Kali-iod is not aggravated from cold.
Lac-can: This medicine is not commonly encountered but when the pains alternate sides no other medicine can take its place.
Mag-phos: Mag-phos is the biochemic remedy for sciatica. But it has its homoeopathic indications too. The pains are aggravated by cold in any form. They are aggravated by cold air and by uncovering the limb. The pains are better by warmth, by getting warm in bed, by pressure and by standing.
Phytolacca: Sometimes this remedy may be indicated not so much in true sciatica but in a pain starting from the hip and coming down from the sides of the thigh. This pain then turns around and reaches the front of leg. The phytolacca pains are usually worse at night. The pains are usually right sided.
Tellurium: Sciatica aggravated by any jerk, such as from coughing, sneezing, laughing, < pressing during stool < stooping. Many times when tellurium has been indicated I have seen either ottorrhoea or more commonly a history of ottorrhoea present in the patient.
The following remedies are much less commonly needed:
Ars-Alb.: The pains in this remedy come on usually around midnight. The pains are burning in character and they are ameliorated by warm applications. There is great restlessness and the patient has to get up and walk around to get relief. The pains are aggravated from lying on the painful side.
Bryonia: The pains in this remedy are better by lying on the painful side and by cold applications. Motion, sitting up and walking aggravate the pains.
Carcinocin: This remedy is only rarely needed in Sciatica but it has come to my aid a few times so that I feel it would be wrong to exclude it. Sometimes the constitutional symptoms assist us in giving this remedy. I have found it work in sciatica where the pains are extremely severe. One of the patients had to move the limb continually for relief; another was slightly better by getting warm in bed; another was severely aggravated by stretching the limb. I wish I could give better indications but I can't, but if anytime you get a patient with violent pains and no other medicine seems indicated or the indicated medicine only partially helps this medicine can be thought of.
Nux-vom: The pains in Nux-vom are aggravated from cold and better from warmth. The pains are accompanied with chilliness and better if the patient lies on the painless side. The patient cannot lie of the painful side. At times the pains extend upwards. Like Tellurium the pains are worse when pressing at stool.
Palladium: This remedy has a pain that extends from the toes to the hip. It also has a similar pain in the upper limbs that is from fingers to the neck.
Rhus-tox: The pains in this remedy are worse from lying down specially lying on the painful side is unbearable. The pains are worse when first rising and beginning to walk but better from continued motion. The pains are aggravated from cold and better from warmth. The patient may give a history of having the pain after getting wet.
Many times you may find the constitutional remedy covers the pains as well. Thus for example Lyco has a pain similar to Kali-iod and if Lyco is the constitutional remedy of the patient that would be a better remedy than Kali-iod. Remedies such as Carcinocin, Lyco, Kali-c, Lach, Staph, Sulph, Nat-m, Sepia, and above all Medorrhinum, are constitutional remedies that not only cure the acute attack but cure the disease permanently.
