Thanks for submitting your information and your interest in the Genus Epidemicus tool. We hope to soon have this process up on the web in a more automated fashion, but for now are sending out these tools in electronic form.
Attached you will find an excel spreadsheet that has an automated form of the tool, as well as a word document with the static version. If possible, please use the excel version as it is easier to use, and is then easier for us to enter in a database. You will notice that in the spreadsheet (and in the word doc), before and after the Nux vomica questionnaire (labeled PART IV), there are some further questions and data gathering spaces. These elements are NOT part of the predictive tool – your input in these spaces will not affect the scoring or outcome of the actual Nux vomica guide. We have included these because we would very much like for you to fill in these forms, on a case by case basis, and send them back to us, so that we can build your responses into a small database (without any identifying information). If you are willing and able to do this, please save all the data entered, including the answers and outcome of the Nux vomica questionnaire tool, as a new file (one for each person’s case) and send it back to us. This will be made easier when we automate it on the web, but we are not sure how long that will take, and would like to begin collecting this information immediately. Again, we appreciate any time you take to send these in, and you will see a return on your efforts if we can compile enough data to make a meaningful database. Your responses on these forms will help us refine and improve our data collection as we transfer this process to the web.
One of the areas of data we are very interested in looking at during the course of this flu is the set of symptomatology that is unique to this epidemic, and is not commonly recorded or reported by CDC and other public health agencies. Part of our purpose in compiling this data is to present the need for a broader, more inclusive system of clinical data collection during these epidemics. This is a first step towards making that case.
We also ask that if you have any questions that you would like answered on the website, let us know via this email address. We have various plans for additions to the website, but would like more feedback as to what elements would be most useful to users of the site.
Finally, in order to broaden the use of this site, and thus hopefully the data collection we can do, we ask you to email the website address to others and encourage its use. To this end, if you have a personal or business website with an appropriate place to link to this page, please do so.
Note that there are some further instructions and context regarding the Nux vomica tool in the electronic files. If you have any problem with the format of these files, or feedback, let us know.
