The rubric 'appetite, increased, afternoon' brings up some useful rx and apart from the 'sweets' that Tanya mentions I would add 'farinaceous food' as well. Lyc is the classic for this time of day and food desires!

I gave her Sepia for starters, it touched but also produced increased
appetite - that's when the 4-6 hunger symptom came out as she said now
she's hungry all day.

You'd virtually never prescribe on one sx unless it truly was SRP and as you've given a rx, the Sepia, then wouldn't it be best to watch and wait to see if this is just a movement in the case and it continues to move along rather than prescribe again - or has a lot of time passed?
Time modalities need to be really strong to include in a case anyway and unless this has been a theme all along I wouldn't give it too much emphasis especially for someone with eating disorders as this will likely fluctuate throughout the case.
Nat carb is in that rubric though - had you considered the Natrums, the carbon has eating problems associated with depressions.

want to tell you? Embarrassed, afraid you would judge, or feels
guilty, or painful to talk about; or ...? Maybe asking what's the most
difficult thing *about* those issues? (Betrayal? Confusion? Grief?
??) Something that lets her talk about the *meaning* of it, and hints
at patterns thru her life, without having to go into the "sordid
details"?

what you call deep depression, and she calls "fine"? Haughty plus
desperate, sounds like fear of something--what? (I always ask re
childhood, and look for whether or where things changed... Was she a
haughty, fearful, controlling *child*? Why? Sometimes people will
relax and talk more freely about the past, other people, and their
childhood, than they are able to about current issues.) Have you got
an understanding about what she's fearful of?

Did you look for any agg symptoms ? Should not this sx assume
significance only when it brings a mini crisis like picture ? Like all
gone, or at least weakness or even a craving.
In a case, I found a sx for fat desire. But it took me a long time to
understand the connection between the stomach sx and a cold and cough
persisting for seven years in my son. The connecting symptom was a
mapped tongue. Allopathic sites maintained it was asymptomatic and
harmless.
Even Homeopathic sites gave no info. Some thought it as a psoriasis of
tongue.
Only a visit to an ayurvedic site connected the liver and tongue and the
cold and the resultant cough. The ayurvedic physician removed all
fried items and baked items from the food menu plus lemon and grapes for
sometime. That was great help.

harmless.
Even Homeopathic sites gave no info. Some thought it as a psoriasis of
tongue.
Only a visit to an ayurvedic site connected the liver and tongue and the
cold and the resultant cough. The ayurvedic physician removed all
fried items and baked items from the food menu plus lemon and grapes for
sometime. That was great help.
