depression

i wonder if anyone can help me with a case on depression. a 56 year
old executive..who has been extremely successful in his career, but
still feels a failure because he has even more successful colleagues.
loner, not a single meaningful relationship in his life other than
contact with family. is on the phone with business calls 24/7. says he
does not sleep well, but is in bed long hours, tosses and turns
between two and 4 a.m. drinks neat vodka by himself every
night.furtive drinking. lethargic.completely sedentary. quiet. never
smiles or laughs. irritable with wife and kids. does not want any
conversation outside of business calls. absolute no interests other
than work. overweight, all the weight on belly. high triglyceries.
tendency to piles.flatulence. had back surgery in the past.

any help would be much appreciated.

I would get a history--what was he like in younger years? (As a child?)
Are there specific time periods or events after which he became this
way, or is it simply a continuation?

Spot on Shannon!

The focus ought also be on when he went into such a condition- whether
slowly( surely Nux in this case ) or suddenly.

And As always find at least a triad of reliable symptoms to make the
case stand on sure grounds.

This man's lifestyle suggests the path of progress given by Boericke
in his opening lines for Nux V. No doubt many have nailed it on first
sight.

But care should be given to rule out any other Rx if further probing
shows any sx and make sure Nux is not contra indicated in the 'picture
of concern'.
