Dr. Ramakrishnan's book is based on the experience of many years of treating cancer patients. Coming from a family of homeopaths, he was depressed when they failed to save two siblings of his from cancer. In response, he basically dedicated his career to the homeopathic treatment of cancer.
The use of an "organ specific" remedy came about because it proved more effective than simply using the constitutional remedy (although sometimes the organ specific remedy and the constitutional are the same, in which case the chances for success are probably greater).
Repeated dosing (up to ten times a day) he found necessary to match the very aggressive pace of cancer -- simply giving the appropriate constitutional remedy and waiting just doesn't work with this disease, particularly in advanced cases.
Alternation with a nosode is designed to boost the action of the organ specific remedy. Ramakrishnan alternates these on a weekly basis, but I've found in some of my animal cases that shorter cycles may work better for some dogs and cats.
Hope this helps you understand the thinking behind his method. It's not theoretical but experiential. Dr. R. simply hasn't been very good about explaining it.
