Hi Roger,
Maybe you should ask her why she is afraid of her fish. Are the fish going
to do something, are they related to something, etc? That may lead to a
rubric. Another tactic may be to ask her if she fear some other things. Is
her general way of reacting to something that you fear to become indifferent
(if that is what leaving your fish to starve to death is) or is it cruelty?
Is this an emotion that she has felt elsewhere?

Also, you say she "woke up one day", and that at that point she
actually *owned* fish! So what else was going on around that time?
Any precipitating events and/or other changes in her health, M/E state,
or other symptoms? The symptom is impossible to use without a context.
But I hope you'll tell us what you work about about it!
