I don't think this should be a problem in this case, buut of course it
may be.
I was also interested in this question, tested it by giving as my name a
wild combination of accidental letters, and no names for father, mother
etc., but didn't take/have the time to fill out everything and save it
to my computer.
It was accepted without any problem.
Of course, any practitioner could do a flowchart map for his patients to
fill it out via a 'normal' software (You could even work out yourself
such a diagram via Microsoft Word, but it probably wouldn't be so easy
to do with the fields for the patients to fill out).
It presents a useful structure, I think, something to work along. Many
people like such graphs more than writing lines and lines....
As for 'Big Brother is watching you':
of course everywhere are risks. I think many people use surveys or
games, where they insert a name, age, weight or something like that. As
long as you don't give a full name, city, address or whatever, it's
usually not to easy to track you down.
But of course we should be aware, that any data, any path can be
misused. Any mail we write may be stored, checked etc.
Any question we have ever asked in wikipedia, google, medical databases
may have been used to put together a profile, one day...who knows?
Everyone has to weigh the Pros and Cons...
