Of course this is expensive for the patient but they are getting out-of-hours homeopathic help and paying someone for it. Not unreasonable.
The people on the helpline are running it as a business after all.
I always tell my patients to have a list of whatever remedies they have to hand before they phone and all relevant information so as to minimise time spent on this £1.50 per minute number.There is a charge for owning a premium rate number in the UK, too, but it hasn't stopped it being profitable!

Is there any phone service in the USA that has a pay by the minute service?
If so, the fees would go into a general account it seems and the homeopath would
invoice the account for payment. The fee structure would have to include the cost of the
service, of course.
If there is such a service, like a credit card payment when placing the call, then we might
be able to figure out what a 30", 60" and 90" call would cost.

A helpline approach also raises ethical issues, at least for me. I've received calls in the past from Pxs being treated by other homeopaths. How to handle it is always a dicey thing, particularly if you suspect that it's not a "real" emergency but something else going on. I've counseled such Pxs but given no remedies.

A helpline will need some serious thinking and organization. These ethical questions are real
and a set of guidelines would be needed. For example, I think it would be necessary to get the primary
homeopaths information for communication about the emergency call.

The UK doesn't do that.
But it may be better if homeopaths find like-minded and like-practicing homeopaths and share call on nights and weekends with them and set things up as they want. I use water potencies according to 5th and 6th edition and would want a homeopath treating my patients who was likeminded.

That would a different and more specific organization.
Maybe have a sign up with your specifics and look for a few people to
share these emergency calls.
school/training programs can do some coordinating with their students, but
leaves out those who are not attached to such an institution.
But again, how to deal with collecting fees.

In my opinion, the cheapest and most effective way to do this is to have some type of live chat software on your website. It's almost like instant messenger, but more professional. The client can tell you what their issue is and you tell them the fee. Then, they use Paypal (or whatever other method of payment you have available) and then you can begin the consultation. I use LiveChat for this. Also, check out Liveperson.com . It's very convenient because you don't need your own website, but a bit expensive.

The problem with this is that it is not universal. Many people do not have
broadband service and cannot use this feature. I have Skype which is great
and if people have a camera on the computer you can see the other person.
However, this would put a limit on people who could call.
And paypal has similar limits.
We cannot assume everyone is up to current technology.
