Sounds like this person could have what is called renal colic/nephralgia.
There is quite an array of rems under kidney pain, or pain cramping etc....
However, with pain as severe as that, I would be treating it as an acute
initially.
Look for direction of pain, whether lasting or intermittent, any urine
changes that may accompany, etc......
Some well known rems for acute renal colic are, subject to their particular
presentation:
Pareira Brava 1x 5 – 10 drops every 15 mins
Equisetum 1x for acute, and 30 c if more chronic
Berberis
Cantharis
Sars
Thuja
Tabacum
Med
Phos ac
And many more, but these are tried and tested for acute pain.
Lilienthal has a small enough list to choose from p.767.

He doesn't like beer, especially (as a matter of fact he's more a (good)
cognac drinker), but he's afraid to drink, because it will cause more
pain. But from responsibility etc. he could very well be a kali-type.
