questionnaire for a Proving

Hello All,

We're getting ready to do a proving here in Salt Lake City on a very
interesting root from South Africa.

What we are looking for is a good questionnaire for biological systems and
symptoms before and during the proving.

Does anyone know where I might find such a questionnaire or have one they are
willing to share?

We already have a set of protocols but would be interested in looking at
others if anyone can direct us to some.
