From an immediate, practical, palliative standpoint, since eating is difficult, will he drink e.g. smoothies?

You need to get as *full* a current picture as you can!

Mentally / emotionally, how is he?
I'm guessing not great, since not-eating doesn't put anyone at their best. But beyond that -- what is the *feeling* of him, from him, around him? Is it along the same lines as before the first Puls, or better, or worse, or different?

And physically, what has changed or hasn't changed?
What foods will or won't he eat?
What will or won't or does he drink? (water? soft drinks? juices? milk? soup? hot, warm, cold?)

Does he sleep well, and/or is there anything notable about his sleep patterns?
I would run through all of this first, before giving another remedy.
Once you do make your choice, a single water dose is a nice, deep yet very gentle way to go. E.g. one pellet in glass of water, stir well, and give a sip (about 1 tsp, amount not critical) for a dose. With e.g. 200c, there would likely be no need to repeat at all.
