As you may know we had a free online course on Case Analysis in Wikispaces.

Our new case is:
The patient is a 42 year old woman with frequent attacks of migraine. She
has been suffering from migraine attacks since about 20 years ago. She has
about 2 attacks in a week each lasting for 2-3 days. This means that she is
rarely pain-free. Migraine is a hereditary feature in her family. The
attacks began after a period of intense emotional tension. A deep emotional
tie was broken up and she was wrapped in deep sadness. Gradually she
experienced headaches increasing in strength. She changed to an introverted
type of person, reserved and deeply focused on past painful memories.
Migraine attacks are typical ones with aura. Vision is severely blurred
before the headaches sometimes the severity of blurred vision is to such
extent that it is like blindness.
The headache is one-sided; sometimes right and sometimes left. One out of
3-4 headaches is accompanied by severe nausea and vomiting. The quality of
pain is sharp, lancinating and pressing. Headaches are aggravated by motion
and light. The only medication which has been able to mildly palliate the
pains has been Sumatriptan (Imitrex) but she has been advised not to take it
as she is also diagnosed to have a moderate Mitral Valve Prolapse (MVP) and
also family history of cardiac disease. In case of anxiety or sustained
physical exertion, she experiences palpitation, breathlessness and short
sharp pains in chest. Propranolol (Inderal) is prescribed for her to control
these MVP related symptoms. Natrum muriaticum has failed to show any
curative effects.
