Case 6:

The patient is a 52 years old man with the complaint of sleep disturbance since 25 years ago. The problem occurs 3-4 nights per week. He shouts and screams in sleep and then wakes up exhausted and goes into a deep sleep again. In mornings following the mentioned episode, he is extremely exhausted and sleepy. He has noticed the point that when this problem is going to appear, the night before, he experiences a great sleepiness and is forced to sleep sooner as if cannot keep his eyes open. About 24 years ago a neurologist referred him to a psychiatrist having no explanation of his problem (with no MRI or EEG study). He discontinued sleep pills as they did not help. No medication for this problem since then.
About 17 years ago he went through bankruptcy. He could not restore the good days he had with his family prior to this point. He frequently feels sad about the event and reproaches himself because of his failure but says that he has been able to psychologically cope with it. Recently he is experiencing a kind of tingling and formication on the left side of his neck sometimes extending to midline in front of throat. MRI of neck has shown discopathy of cervical intervertebral discs. He relates this problem to his bad posture while he is at his desk. Another problem he complains of is occasional burning pain of stomach. Endoscopic examination has shown mild gastritis; anti-acids prescribed.
