This is SO important - not so much the advice to remove any causation or maintaining cause, that speaks for itself even though in the more modern world this is increasingly more difficult to do - but more the sentence about a chronic miasm lurking and how these 'noxious influences' wake the miasm up, so to speak. It is vital that case taking be of the absolute best and most thorough, to determine to what extent these causative factors are responsible or not, before advice or prescription made. Not as easy as he makes it out to be
