Dear Soroush, I think it is always worthwhile making a distinction between acute disease and chronic disease.
In acute disease many micro-organisms can be in play and the fundamental cause in chronic disease is 3 miasm or 4 if you count in TB. But there are other causes of disease apart from the fundamental, as we know. But it is still about susceptibility because in a healthy body micro-organism cannot harm, otherwise we would all be deceased by now so that says something about the harmfulness of micro-organisms. They are opportunistic on a weakened vital force but you could say that if a body is already weakened (by other factors such as exciting cause, maintaining cause etc) then the micro-organisms take opportunity to that and contribute to the cause of ill health - this is susceptibility. But which came first the micro-organisms or the susceptibility - again worthy of reading what David says about this at the time of Hahnemann there were various manners of thought about this. Also Hahnemann, as we have seen recently, in § 11, where he writes about the conceptual force of disease, you can stand next to someone with an infectious disease and it may invade the body but if the vital force is healthy it will do no harm, but still be present - but this still pertains more to acute disease rather than chronic. In chronic disease today, the micro-organism might not be present in actuality but the weakness that was endured in generations before have set up the susceptibility for numerous diseases that pertain to that original infection - nothing to do with actual presence of micro-organisms but this is not to underestimate the power of the inherited miasmatic trait.
Susceptibility is paramount as without it we have a world we know nothing about, can only speculate about.
