Is there any mention as to his reasoning? Clearing/bring accessory symptoms forward, etc.?
Perhaps due to limited transportation and no phones?

No - they are just notebooks. Case records. For his own information at
the next and following appointments.
The longer part probably yes: patients may have lived far away or gone
on a trip.
Shorter probably - I did not do any research into this aspect -
because of rather acute problems in the course of a period of
treatment for a chronic disease.
