I'd like to hear your views on rubrics (or any other comments or
suggestions you'd like to make) for this interesting case. I have seen
a similar case a couple of years ago with a man in his mid 40s, going
through the penile kundalini psychosis but had to refer him out. I
have also seen a man in his early 40s who looking back, seemed to be
at the beginning of the psychotic episode. This is the first time I
have seen the end of it with this set of circumstances. I am posting
many details and hoping that some rubrics arise that I may have
missed. You may contact me off-list if you like, I know this case is
very involved. :-)

Hi Christina,
I do not see it so very much involved - so I am probably missing
something:-) But anyway:

There was *trauma to soft tissue* in the past, in this case the
kidney, which probably left the kidney somewhat damaged and prone to
react to strain on the system.

So now you have as primary complaint *burning pain* in the *kidney
region* *aggravated by intercourse*

Since it is also aggravated whe he smokes marihuana, this smoking does
something to him, psychologically or somatically that also stresses
the system, as intercourse does. It may be the general strain (you
would have to find out whether anything else aggravates) or it may be
related to what is going on at intercourse, e. g. the build-up of
tension and sudden release, the emotions involved. You would have to
find out whether there are any similarities in his experience of
smoking marihuana and of intercourse. If nothing comes up with the
questioning, you might want to ignore the aggr. from the marihuana.

Then there is that history of constant erection of penis. One might
look for "priapism" or look at the related rubrics in the reps. (I
should do the latter).

One will have to see whether there are remedies that cover all of the
above symptoms.

If so, one would have to look them up in the MMs and see which one of
them fits best.

If there is no remedy that covers all, one would have to look
through the MMs of the rx that cover some of the sx and make the same
decision.

Only if that turns out to bring no results would be at the "scratch
head" stage and start afresh.

Since you did not say what you have done so far with which results
there is nothing else I can say about it.

Hi, Christine --
I'd agree with Luise's approach. (Luise, what's happening here?!) It may even be that a simple trauma remedy will fit the symptoms.
It strikes me, though, that the patient has told you a lot of things, but that there's no real indication in what you've written that you've had an opportunity to ask him questions. What seem to be missing here are:
• modalities on the kidney's burning pain once it starts;
• knowledge of what led up to the "kundalini psychosis";
• what he means by a gentleman's club and how it's relevant;
• when he began smoking marijuana (does it relate, for instance, to what happened at the age of 44, or can we rule it out as a possible chemical cause of certain troubles?);
• source of the body odour, and whether he is as conscious of it as others are, or even moreso;
• timing of the mental cloudiness / fogginess in relation to eating, drinking, and sleeping patterns and intoxication (a journal may be necessary here), though its common appearance after "detox" may suggest the possibility that "detox" is affecting his electrolyte balance adversely;
• "problems with yeast" may hold some particular symptoms worth noting (and all wives are meant to have vaginal yeast; it is not an infection!);
• other symptoms.
With a patient prepared to volunteer a lot of information, perhaps at a fast pace, it's probably going to be necessary to spend a lot of time in order to let him talk out all that he has to tell you before yourself having the opportunity to ask him about completely undiscussed matters. Once he's said his piece, you may need another three or four hours to ask him about those things that you want to know -- longer, if he answers at length. And it's probably going to be helpful to be systematic about that, basing it on directions such as those Hahnemann and Kent give as to the enquiries that are useful.
The traumatic approach may be all that is required. But the mental fogginess, the drooling, and any other symptoms you don't yet know about are likely to remain untouched by that, and once you've begun a course of treatment it's going to be difficult to learn what you could learn now. Even if all he ever does is have a traumatic remedy and see the related problems disappear, for everybody's sake -- especially his, if he chooses to have further homoeopathic treatment in future -- it's valuable nevertheless to have on file a complete record of his present state and history. You'll never have this opportunity again.

I have thought of one more symptom that might possibly be relevant:

Here I would stay with the phenomina and not bring in my own
interpretation, i. e. that it is/was a "psychosis".

Unlesss he is telling big tales, (which is quite easy to do as far as
tantric practices and tantra itself are concerned, since most people
know nothing or wrong things about it,) he got to be pretty far
advanced in the practice in order to be able to do this "retrograde
ejection".

This would have meant quite considerable strain, both physically and
psychologically. So I would add the possible symptom of
*physical/mental exertion aggr.*

The other symptoms below and all that may come up further I personally
would leave out for the initial step and only keep them in the back if
the repertorisation with the ones I mentioned does not lead to a
satisfactory result.

This is where we differ, John -- taking into account your other mail
on finding the simillimum. Mine is one of the different kinds of
approaches JROZ talked about. My approach as a rule is to get the
symptoms, where I follow more the line of Hahnemann, whose initial
case-taking in his journals rarely takes up more than half a page -
although mine might be longer than that. Then I would search for the
sx that seem relevant, group them in a hierarchy of importance and
then do my repping and checking the MMs.

Doing my repping still by book or rep in the computer, i.e. not by
Radar etc., does play a part, since doing it your way would take
weeks:-) But I do not believe in punching into the software everything
except the kitchen sink, in any case.

It's just my own personal view
on the matter, not meant to criticise the other ways of doing it.
