A man 45 is looking for homeopathic treatment for his chronic neck &
shoulder pain. He has severe pain once every few months often on wet
weather. He treated this with Aspirin when it happened. Over the
years it gets worse. The most recent attack happened a few days ago.
Aspirin was no longer working. After several hours of brutal pain, he
has no choice but to have the pain managed by the emergency room. All
tests said that nothing was wrong with him.
After taking his case, I have decided to use Pulsatilla as his
constitutional treatment. I am looking for suggestion on supporting
remedies before starting his treatment. If his severe neck & shoulder
pain returns during constitutional treatment, what remedy do you
suggest to use to manage the pain without changing the direction of the
healing? Thank you in advance for any input.

You may wish to study Dulcamara which is < wet weather and stiffness in the neck with cold.

I would look closely at other modalities to find the best choice - such as type of pain (stabbing, throbbing, burning, aching, cramping), what else makes it better or worse (heat, cold, laying down, pressure, noise, walking, moving, time of day).
See if you can determine the causality based on symptoms - a never well since, synovitis, post-herpetic neuralgia (such as post shingles), gout, stenosis (narrowing), bone degeneration, spasms, etc. This can help in the selection as well.
Also consider remedies which should be compared to Puls such as Kalmia (neuralgic pains in neck and shoulders worse becoming cold, cloudy weather - also follows Puls well) and especially Spigelia (tearing pains shooting into the shoulder and neck; neuralgia, pains like being stabbed with a knife or needles, wet weather agg.; also follows Puls well but is antidoted by Puls).
Perhaps it is a NWS of repetitive grief, death of parent or friend, hearing bad news, losing sleep while caring for someone, or excessive cortisone. I would then look at Causticum which has neck and shoulder pain and pain in wet weather and follows Puls well.
Additionally, with water dosing, you can stop the chronic remedy and dose with the acute without worrying about slowing/spoiling the case.

if after studying the case you believe his constitutional rx is puls
then that should take care of the chronic pain that he has, he may
still have it but it should be manageable. if it is not relieved then
you should take a look at your second choice rx that may have been
close but not it. if you use classical homeopathy i dont think you
should mix rx it will muddle the case. stick to basics i found it
works the best...kali
