Although I can't show the rubrics to prove it the first thing that came into my mind was Stramonium. He is VERY fearful and lashes out in anger. What is the main sensation? Fear/anger? Lyssin that Jean mentioned would need quick remorse after the anger which you didn't mention. If he is hearing voices and the moods are changeable it maybe worth looking at Anacardium.

I didn't take the time to combine the similar rubrics, but
Repertorisation of the rubrics listed below brought up: Bell,
Aconite, Phos, nux v, lyc, sepia, stramonium, cocculus, hyoscyamus,
petroleum, silica as the top few - nightshade family is definitely
strong.
I wondered whether the car travel might aggravate the vertigo, hence
the need to escape from the car.

It also sounds like circulation issues - The heart problem, and brain
issues - vertigo, left eye and left ear could be related to lack of
blood flow. If this so, an organ remedy for heart and circulation may
calm things down a bit - so: Crategus (top choice in mother
tincture), Strophanthus, Cactus, Adonis vernalis, Convallaris or even
Baryta Mur, could be useful either in mother tincture or very low
potency, 2 or 3 times a day.

did you give it that way (in dilution as per 5th Organon). Did you do

dry doses were used at the time as dilutions seemed to aggravate.
perhaps this is the time to try dilution doses.
