I have a case in which the boy (age 6) loved to dress in mom's undergarments. He also would follow mom everywhere and whenever she would shower or use the bathroom, he would run in to look at her "parts." He liked to pretend to be a girl. It was really becoming obsessive. He's quite a charming little boy - very sweet usually. Diagnosed autism but is more likely aspergers. Has a younger brother.
Farokh Master's Children's MM lists under Hyoscyamus:
Gender identity disorders in children: Desire to pretend to be the other sex, preference for cross-dressing in boys, girls insist on wearing only stereotypically masculine clothing.
Also,
Envious and jealous children
Violent outbursts of temper (although this child had the suppression of anger discussed in Massimo Mangialavori's Solanaceae text)
Ailments from being alone or emotional excitement
Behavioral problems and conduct disorders in children (kicking, obstinancy, quarrelsome), temper tantrums. This state is especially seen in cases where the child is very jealous of a newborn sibling in the family and desires to hurt or injure the infant in order to get back the lost attention from his parents.

While we don't label in homeopathy, I talk with a lot of parents with kids on the spectrum - these behaviors are very common - difficulty with transitions and the resulting tantrums is extremely common. My son had this and it was like a delay in adjustment to the new activity. While he was (is) incredibly bright, it was a processing issue. He would also wave a few minutes after people left. The tantrums were outrageous. Part of it is also the extra sensitivity (the label is sensory integration dysfunction or disorder).
In my son's case we traced the behaviors to a phenol intolerance (could not process phenols from most fruits and vegetables). The behaviors were eliminated using an enzyme called NoFenol. Other physical symptoms were red ears and red cheeks. However, the need for the enzyme was eliminated after he took the Thimerosal remedy (which is a story for another day and not something I recommend).
So while we like to look at behaviors according to mind symptoms, sometimes they are indeed physically induced (not mentally induced) symptoms. At that time, we were doing occupational therapy and did the whole preparing for leaving, 5 minute warning, 1 minute warning, time to go, go routine. It helped some but honestly not much. Thought this might help you in understanding the behaviors in some way.
The boy from my case did quite well on the Hyos (had many other indicators for it) and all of the obsession with the undergarments and mom's parts stopped (it's been over 9 months now). He eventually went on to Calc-C which he is doing very well on.

Oh another thing I forgot...
You mentioned in your case, "he tries to be good" but can't.
In this boy's case, he says, "My good part ate my bad part and now I can't be good anymore." Or "I was good but now my bad part took over."
This (with other things) eventually led me to Stramonium and then Calc-C. On the Calc-C it stopped and he said his "good part" was back and eventually stopped mentioning it. His expression of anger is much healthier now. I personally think his anger was so suppressed, the Hyos allowed him to feel his anger for the first time and it completely freaked him. But definitely was some type of split there:
mind, delusions, divided into two parts

Thanks, Erica and Teresa -- very helpful insights -- especially, obviously, it's important to look at potential physical causes for what seem to be mainly emotional symptoms in these kids.

I found your 2 posts very interesting because in the case of my patient, who also had behavioural problems I went from Carc to Hyos and then to Stram which was the last Rx I gave him over 2 years ago!! If he ever returns I will have to think of Calc

i had a young man several years ago. he was very effeminate
and very attached to women. his father was hostile to him and
his mother pretended to support him but made him feel guilty
about his female expressions. i dealt with him with his ADD
and school issues (the school's racism and homophobia was in full
bloom), and tried to calm the mother down about his female identification.
the boy tried to shift into male behavior but he finally came
out strongly when in high school. the last time i saw him he
was a gorgeous 18 yr old, confident and comfortable in himself.
i am assuming his mother made peace with who he was. he was like
this from infancy.

Hi, Sue.

Interesting remedy! Yes, it resonates with a lot of the case. Not sure
about the tantrums, however, although these are a reaction to feeling
powerless, unable to control the environment. Also, interestingly, the one
physical pathology has been a growth that bled and bled, and never
coagulated, necessitating its removal -- sort of the opposite of what one
would expect with a snake remedy??? Or would it be considered a polarity in
this case?

Hi, Vera, I thought about Anac. But any hardness is lacking, and there have never been any skin symptoms other than the nevus -- other than the very common early neonatal eczema/ whiteheads on the face, which was not suppressed, and went away leaving no aftereffects.
Also, regarding snake remedies (Sue's reference to Python, which I hadn't studied before), there are no apparent bleeding disorders, other than with the nevus.
Interestingly, I've just received the pre-intake questionnaire from a prospective adult new patient who appears to have had exactly the same profile of symptoms (minus the nevus) between ages 3 and 4 as I've described for this little boy. Hmmm....
Erica, your description of your daughter's early history would reinforce the idea of looking again at the solanaceae. Thanks.
