Is there a rep for...?

Post by McPhee Family »

sexual perversions? I have a client with some perverse issues which seem to be a major part of the case. I haven't a clue how to reperatorize or if there is a general rubric for that sort of thing?
2nd question (unrelated) - is there a rubric for a weak stomach listening to medical issues?
Thank you!

I'd like to follow on Erica's first question, and ask what rubric(s) people would think of for a pre-school aged boy who loves to wear girl's clothing sometimes, and tells people that he's a girl ("Hi, I'm Sophie." for instance). Sometimes he's himself, happy to wear boy's clothing. His play interests are typical boy ones -- cars and trucks mostly. So this isn't the whole case, but it could certainly help in deciding, for instance, between Cham and Med. I guess that confusion and indecisiveness about everything is at the core of the case. So MIND-confusion identity, and confusion sexual identity would be one place to look. Other suggestions?

Without knowing anything other than your description - is it possible he simply enjoys pretending - and that there is no confusion or sexual identity problem?

and why assume confusion. he may be trying on different identities
which is typical for his age. also, he may be enjoying the females
in his life and wanting to experience their energies. i would not
assume confusion. and don't forget the possibility that he may be
a female identified male as his core being. i would not mess with
that. if this is the case, you can really create some serious
confusion and misery for him.

Hi, Tanya.
Yes, no one's messing with his frequent desire to be a girl -- amazingly. It will pass or it won't, and the people in his life seem reasonably cool with that, at least at the moment, or they're keeping quiet about it (I expect problems there could develop as he gets older, like school-age....). Right now, the big issue is really the constant indecisiveness and tantrums that result. I think it's going to be a choice between Carc and Puls, as I work on it and think about it.
Thanks. Rosemary

Dear Rosemary,
It may well pass. I had a patient a few years ago age 4 who used to be like this and now he is 10 I think he would be quite horrified!! His mum just ignored it. I note that he had Carc and Hyos when he was little and age he got older seemed to do well on Stram. I haven't had him in as a patient for over a year but have seen him when his younger sister has been to me. He is the one from years ago who always made "cages" or "fences" around toy animals and we never got to the bottom of it. The last drawing he did for me was of Medusa and put my name under it!!! It is still on my wall as if he comes in he will expect to see it there with other childrens' drawings!! At least when he came with his sister last time he added in pen under it "but she is great" . A strange one!!!!

Thank you Didi and Jean,
Didi, those are great rubrics to have. I appreciate you taking the time to share.
Jean as always - very insightful! Three of the top choices for this one. :-)
No one on the sexual perverse stuff, huh? It's surprising, I would think that would come out in provings or be SRPs.
Truly,
Erica

Dear Rochelle.
Thanks for the words of experience. His dressing up may well pass, and it's good to know of someone who has experienced this :-)) I loved the comment he added under his drawing of you!
This boy's frequent tantrums are more of a concern, frankly, because they make him and his whole family so unhappy so often, and make it difficult for him to be in pre-school or go out with his mom and brother -- they try to avoid occasions when the tantrums would occur.
In preschool classes, he stays by himself or hides behind Mom, if she's there, until almost the end, then when it's time to go he melts down in a screaming tantrum because NOW he wants to do whatever they had been doing that he hadn't participated in, and it's time to go. Likewise, when they're getting ready to go somewhere, he doesn't want to go, then when they change plans and decide to stay home, he melts down, shrieking and kicking and wailing, screaming that he wants to go. But if they then get ready to go again, the scenario repeats itself. Irresolution, certainly, and also anxiety -- about performing? About going out? Anxiety about new undertakings, anyway. Contrary?? But not in other circumstances, so probably not. When it's time to go, Mom generally insists they go, which is undoubtedly good in terms of his learning about the limitations of time on such social occasions. But it's not pleasant.
Mostly I see a little kid who had his mom to himself until the brother came along a year ago, and now he has to share, and he has tried to be "good" but can't manage it.
He's also very clingy to Mom -- I forgot to say that before. Mom wishes he'd stop needing her right there to touch at every moment. He does sometimes try to behave like his little brother, which is probably fairly common as a strategy to get back mother's undivided attention. When he does that, it sends the little brother off into gales of laughter, which this child also enjoys as a way of being at the center of attention.
The alternation between the sweet, cooperative child and the one who desperately, terribly wants something he can't have or get and doesn't know how to articulate, is an important part of the case. Crawling into Mom's lap when he calms down a bit is the only reliable calmative. Sometimes another lap or someone else's attention can do -- but not always.
He's very sensitive to stimulation and gets more and more excited, with no way to stop himself. But it's endogenous -- he's not being malicious or mischievous, or hurtful in any way, and he's not willfully destructive.
He's an extremely earnest, frank, open, thoughtful child in the sweet phase. No deception, grudges or lasting ill will at all -- when it's over, it's over, and he's back to his sweet sunny self with no apparent emotional after-effects. His "alter egos" -- the girls -- on the other hand last for a week or two at a time, just in regard to clothing and the name he adopts -- no female mannerisms, etc... (although he is quite androgenous looking).
I think there's a strong case both for Carc and for Puls. The sweetness and clinginess both make me lean a little toward Puls. The facial nevus is in Carc, not Puls. He likes just about everybody, adults and kids, and enjoys being with people. Hates to leave the room unless somebody -- anybody, not just Mom -- goes with him. If encouraged to go by himself, he decides not to go anywhere, not even to the next room.
Although hurried, he's not at all awkward -- he's advanced for his age in neuromuscular coordination and abilities. It seems another one of those cases that bring to mind several remedies, but not without contradictions -- which also could indicate Carc, of course.
Rosemary
