Does anyone have any ideas on this symptom complex?
Dry wheezy cough, brief spasm, followed immediately by inability to breathe, loss of voice for a minute or two, subsequent hoarseness for about 30 minutes. Cough is accompanied/followed by slight burping.
I've given Phos, which seemed initially to help but symptoms returned. Gave Bryonia, which has caused some improvement but there still is coughing although the burping seems to have gone. Thinking of Caust. if improvement doesn't continue.
Talking agg. Any kind of spray or strong odour agg. No history of asthma. Cough spasm can occur at any time of day, not frequent but when it does happen it's very unpleasant. Has also happened during sleep.
Nothing clear on emotionals except perhaps increasing frustration with his job where he is not using his full potential.
Any suggestions welcome!

In our experience the cough you've described Spong seems to be indicated. E.G. Dry, Cough followed by SOB + hoarseness [=hoarse cough]. It can be tried in 200cH potency QDSx 5/6 days.Feed back will be appreciated.

Could it be related to reflux,??asafoetida. Not in rubric I have of talking aggravates but has reverse peristalsis spasm of gullet and burping. Jean

I searched Encyclopedia Homeopathica for this particular combination
of symptoms: cough with hoarseness and eructations. The results
indicate Ambra Grisea: "Frightful spasmodic cough, with much
eructation and hoarseness – (Hahnemann – MM Pura). Hering: "Spasmodic
cough, loses her breath" and this is aggravated by talking (Guiding
Symptoms of Materia Medica). Kent says that a good deal of this cough
is of nervous origin. (Lectures of Materia Medica). I'm not sure if
this matches your case but thought that maybe it's worth having a

Thanks for all input.
It started about 3 weeks ago, possibly connected to change in weather. He's never had this kind of cough before, usually coughs go further down into the chest there's more expectoration. Could happen 2-3 times a day, although more if he's doing a lot of talking. He doesn't cough every time he talks, only sometimes he has to give presentations and is likely to have this kind of cough after presentation.
No connection to food or drink, no reflux, people tend to give him cold water which makes no difference, nor do hot drinks. It's a wheezy cough - I always understood whooping cough or Spongia type cough to be more barking. Ambra doesn't seem to fit the picture. For some reason this patient does not respond well to 200C, even in acutes. I've been treating him for a while, and he seems to do best on potencies from 9C till 30C and LMs. Haven't tried him on 1M yet...
It has happened at home, outside and in the office, but it was noticeable that when he had sick leave, it started to go away, went back to work and it came back. The cough has also woken him from sleep. He works in a large office, hundreds of people, and I've recommended that they make sure their airconditioners are cleaned out properly especially before winter. He also has increasing frustration at work, together with increasing involvement in work, which was part of the reason I went for the Bryonia, and there has been some improvement since he started taking it. He gets very frightened when he can't breathe - possibly a case of "doesn't believe he'll get better?"
