Hello all
I have a patient who is suffering from chronic recurrent diarrhea
(celiac sprue), He is an Asian from Pakistan

His Antigliadin antibodies IgA is positive and Antigliadin
antibodies IgG is normal, Anti transglutaminase IgA and IgG are
normal apart from that all other reports are normal.

the history is he had recurrent bouts of diarrhea which is painless
always, he has stopped taking wheat products but still he feels
uncomfortable and there is sudden bout of diarrhea which causes him
severe weakness that he cannot even work properly, there is Diarrhea
even with anxiety (according to him when traveling or even going to
any engaement he used to first go to the toilet pass some stool and
then go out, reason being uncertainity of what to do if it comes in
between the event ), there is rumbling in abdomen, Severe weakness
after diarrhea even unable to work.used to eat wheat products
without any severe complications except he used to have 2-3 soft
stools in a day and now he has been advised to stop eating them.

History of bouts of Diarrhea on and off but not bothersome, the
condition had aggravated since 5-6 years and now increasing in
intensity

allergic to dust causes watery coryza, coryza worse cold or change
of temprature,

vertigo with ringing in ear constant. Vertigo started when he went
to Niagra falls Canada head became cold went to the toilet and dried
his head with dryer and again came back out and had to stand in the
cold for sometime and from next day the vertigo started it was in
1998 and since then it has incresed in intensity,sense of losing
balance when standing, vertigo is also worse when attending some
meeting or going to work and gets better (is still there in low
intensity) after the end of the event or when at home, feels better
at home, fear of going out due to vertigo

Low in confidence, Age 34 years, hairs greying, black circles below
eyes

family history from mother side tuberculosis and Heart problems,
from father side Cancer diabetes and hypertension

used to eat a lot outside spicy, farinacoius food, pizzas but had
stopped eating them now.

Also palpitation during exertion and also during any event which
gets better afterwards
this is a short history, anything else i will be happy to give, will
be waiting for a good advice
