ofcourse it is reasonable to make a full case taking as joy said but
if there is no other signs (as may be in one-sided diseases) it could
be worked on local symptoms as well esp. when so striking are as this
mez can be a choice if there is not any other symptoms

This doesn't read like a one sided case at all. There are plenty of sx
to use imo and could represent any one of a very large number of
remedies. Only a full case taking will determine mezereum or not. I am
only reiterating this because it really could be any one of many rx.
Just count the rx in the relevant rubrics.

You would expect peppers to burn in the mouth so don't be entirely lead
by the few rx in that sub rubric - the simillimum could be in the main
rubric. The other modalities such as << something cold like ice cream
is more curious, i.e. a burning that is not alleviated by something
cold.

The simillimum could be anything from Cantharis to Tarentula. Best

I agree this is not a one-sided case.
Just the burning pains might be too general... but when the patient
qualifies it as if it is like pepper ( instead of say like as if it
is fire) then it becomes important - should not exclude others but
the ones that feel like pepper are more important... burning <
icecream ( how about cold drinks?) is peculiar enough to carry the
prescription with the locality...for the current complaint...
esp.since the patient is not that convinced about chronic
homeopathic prescription. Since burning mouth syndrome has no
concrete aetiology allopathic treatment is going to be frankly
palliative. So any help we can provide is going to be a blessing.
