headache

Dear All,
I have a patient with a headache which she has had every day for the past 2 weeks except for at the weekend. I am having difficulty getting much out of her as she doesn't really want to talk. I think she is still working through the headache. This is all new. She had a Sulphur 200 14th July which she has had before with no problems. On 18th July she had some cystitis and took a Cantharis of her own accord. I have studied sooo many Rx and can't find a suitable one. I am obviously missing something? What other questions should I ask.? (I have sent an email tonight to ask about if it is sudden or gradual start/stop)

Symptoms
Until today it started after lunch. Today it started earlier in the morning tends to lift in the evening.
Constant awful pounding at the top of my head, slightly to the right,also at the back of the head.
Irritated by loud noise and light
Today she feels she wants to lie down but not usually
I don't want to communicate

Hi Rochelle,
I couldn't help but wondering if your patient's headaches are
work-related (stress-related). Perhaps they come from mental exertion
that reaches its peak at lunch? Could explain why she doesn't get them
on the weekend... What's her occupation; what's her day like? What are
her thoughts before the eadache kicks in? Maybe she spends the day
with someone who causes her to be very stressed?

Have you seen her when she is suffering? Are the pupils dialated? Are they dialated evenly? Are her lips blue?

Possible things to ask her about:
Does heat or cold pack help?
Is this related to a hormonal change? (such as pregnancy)
Environmentally - is there a new cleaning chemical being used at work? or in the place she eats lunch? or construction in the area?
or have the lights recently been changed ( for example to florescent bulbs)? or is there a new machine in the office? Has it been particularly wet lately (possible mold growth)? Has she changed chairs at work - or computer screens? Had any physical accidents? Changed perfumes, toothpaste, or skin care products lately?
Food - has she recently changed vitamins? morning food? mid morning snack food? beverages? sweetner? etc.
Checking with a common 'headache trigger foods' list (such as citrus, cheese, red wine, grape juice, some nuts, etc. ) - have any of these been on the menu lately?
Is there a relationship to recent world events (such as the London bombings?), or family events?

Of course, weekday stress is a possibility - but unless it is suddenly unusually stressful at this time - stress would have caused more of a 'build up' type headache as opposed to something that starts full on only two weeks ago.

One more thought - Any sudden onset violent headache needs to be assessed medically.

I knew of a case where the man only had H/A's on Fridays. I was thought
to be a culmination of the weeks stress.
Turned out that he had started working in the payroll dept and they
filled pay envelopes with cash on Fridays.
He had a mild allergy to nickel that only manifested when he handled
excessive amounts of nickel on Fridays soooo
consider work related factors.

I think needs blood pressure and temperature checked. Coldness can
indicate a fever. Also urine should be cultured.
Maybe the UTI did not resolve completely
Why the neck stiffness. With the sensitivity to light and noise there
are serious possibilities

What is she eating and drinking at lunch (watch out for aspartame and MSG)

Also has she had her eyes tested?

Is she sleeping enough?
After lunch there is a natural tendency to sleep and sadly most people -
including myself) work through it. Lack of sleep can lead to Headache.

Dear All,
I will try to answer everyone's posts in one.
My patient is the osteopath with diabetes with the dreams of being pursued which stopped when she was on holiday in India. Her exhaustion is more likely to be a physical exhaustion because I think her job is very physical and she can have a lot of patients in a day.

Thanks Donna for you list of questions. This has happened since she returned from India and therefore we think work related. I will ask about if she has put any of those nice smell things in her practice room which may be setting up an irritation in her. Good thinking. If it wasn't for the weekend without the headaches I would also agree with you about her being medically assessed. She doesn't like allopaths as she thinks she knows more than them re her diabetes.

`Thanks Venkat- it is Nux that to me is the closest. She took one yesterday but after the headache had started whereas I would have preferred her to have taken it when she was free of the headache in case it got used up more quickly.

Thanks Soroush. I have a running battle with her against Aspartame as she drinks the odd (so she says) diet coke. On the whole she does have a healthy diet.Her sleep is punctuated with the vivid dreams and I am not sure where we lie on this at the moment but I will find out.

Dear All,

The latest on this is that she has phoned me and we went over all the point and questions you suggested and basically came out with nothing new. The only thing she thought of was if it could have been related to the food poisoning she came home from India with although it was a couple of weeks after that the headaches started.
This morning she just has a tension feeling in the vertex and occiput which she feels is a precursor to a headache. Unfortunately she took a Nux 30 this morning (she has her own Helios kit!!) when she woke after taking the 200 after her headache had started yesterday!!!

I am going away abroad tomorrow and am anxious to sort this one before I go. I read the MM on Silica that Venkat posted and she thought that most of it sounded very similar. She does have Silica 30 and 200 in. I will also make sure that she has some more Nux 200 before I go. I will speak to her this evening when I go round with them and see how she has fared today
