Essence help?

I had a patient in yesterday and the essence of the case which he kept on repeating was "I don't think I am good enough and people will find out.". This has prevented this 55 year old very loquacious man who knows about and has tried every complementary therapy and counselling you can think of, from have a relationship with any woman or take on a new job or challenges because of the worry about this. "I've always held back." Physically his back is in a constant tension and he doesn't think he is standing tall. "I've been told I analyse too much and there is too much negative talk." He says he talks aloud to himself constantly. "It's a fear of not being good enough." This last phrase is repeated time and again. There is depression and sadness with all of this and Grief when his Mum died 6 years ago.

What sort of Rx should I be looking at here?

Grief is common to us all
Depression and sadness comes to many
Lack of self worth hits most of us at some time
Holding back is a rational result of lacking in self worth

More curious is that this lack of self worth will be witnessed by others

Particular to him is the tension in back and not feeling tall, analysing
and talking to himself ­ very important.

But that aside, as this is not what you asked for, I would try to draw a
line through the case and below the line would place, if appropriate, the
utter hopelessness and self condemnation that spirals towards a syphilitic
state, where the depression becomes black and suicidal eventually, where the
grief cannot be overcome and the internalisation (talking to self) becomes
complete, e.g. Aurum. Aurum is in the rubric - lacks confidence and others
have none which make him unhappy.

Above the line, if appropriate, I would place the more psoric counterpart,
where the lack of confidence and grief take the form of anxiety, timidity
and introversion and perhaps is heading for a total loneliness where other
compulsive acts, other than talking to the self, take a hold such as
alcoholism ­ these compulsive acts are there purely to fulfil a lonely need.

Is the case moving towards destructive torment or contemplative
introversion?

I suppose this isn't calc, fearing others will observe his condition (or
whatever the phrase is)?

may give you an idea of a direction to pursue:

In my opinion, the avoidance is an important factor. It releases
him from responsibility to perform in a relationship or on a job,
for fear of failing at either or both. One runs no risk of failing
when one fails to risk. His lack of self-confidence keeps him in a
state of doubt about himself and his abilities; his avoidance of
situations in which he may fail to live up to his or others'
expectations is a defense mechanism. In his state of suppression,
his tension is somatized. At the risk of extrapolating,
his "stooped posture" reflects his image of himself and his
inability to "stand erect".

This brings up a good point, as Anac was one of the remedies that
also came to my mind, but I rejected it, since Anac does not
typically practice avoidance, but rather pressures and works itself
ruthlessly in an effort to prove itself.

P.S. I'm not sure that I've understood you correctly, as in your
last sentence you mention that what you have quoted "rule out"
Anac. Is that a typo? Was it meant to read "don't rule out"?

main essence and keynote of Anac. as taught in late 80's. It is not
enough of course on its own, but definitely rule out anac.

further, and could be found by asking Why? and then What happened? and then

good enough? If so, then how does he do this? What kinds of activities does
he engage in or behaviours does he adopt to do this?
This could be found in the rubric

If he does cover up, then that also would lead one to suspect Sycosis

also

if people did find out what would he fear would happen?
eg.
Fear of humiliation
Fear of opinion of others
fear of people
fear of women
fear of being looked at
does he cry?
Does he feel helpless?
has tried
relationship with any woman or take on a new job or challenges because of
the worry about this.

he doesn't think he is standing tall.

***he gives his BACK tension cause away by saying "I've always held BACK"
(he would probably be a good candidate for psychotherapeutic bodywork :-))
This somatisation can be found in the Ranunculaceae family according to
Sankaran.
If you have this information Rochelle, I would read it.
Eg., look at the summary of Miasms at least at Sycotic, and then at the
sensations of Ranunculaceae and see if you find a correlation.

One that jumps out from this method of investigation is Pulsatilla.
On page 795 of the Plants book Vol.2 he says:

"When the sensation of the Ranunculaceae family is combined with the sycotic
miasm, the feeling of Pulsatilla could be: Avoiding situations which causes
vexation"

and

"Pulsatilla hides its anger and irritability out of fear, due to a feeling
of dependence. "I am not adequate in myself and I am dependent and so I have
to cover up".

The fact that he has tried everything in the form or therapies to get help,
could be an indication of 'dependence' I suppose. Look at rubrics like
and (Puls in both)

I am not trying to say this is a Pulsatilla case, however, might I mention
that this remedy does contain rubrics such as:

want of self confidence
fear of failure
ailments from failure
fear of being humiliated
fear of people
fear opinion of others
fear of women
timidity
weeping when looked at
feeling helpless
back tension
Depression
Loquacity
desire to hide
Yielding
Discouraged
etc....
Staphysagria is also in the Ranunculaceae family and has some similar
rubrics as Puls.However, using Sankaran's ideas, he places Staph in the
Cancer miasm, which does not seem to fit this persons presentation or what
you have given to us in info so far.

Another idea, I would take a look at the AIDS remedy which covers alot of
what I have mentioned as ell, and maybe Nat mur.

Just some ideas from my rambling mind

Thanks to Toni,(you came up with 1 or 2 rubrics I didn't find!!) Joy, Andy, Shannon and Praveen for your interesting contributions. You have all given me a lot of food for thought. He was a pathetic little man if that helps.

Aurum to me is a go getter, someone who is determined to make it to the top and gets depressed when he comes second. This man wouldn't even try because of his low self confidence. I don't see Aurum as a remedy which would "hold back" but one that keeps trying. I will look at Aurum Mur , thinking about Mother as she may well have had a big influence on his life.
Anac - I will have to study as I don't know it very well. Ambra gris is interesting. I will have a good look at this one tomorrow.

Yes Calc does have that rubric Shannon but I don't see him as Calc.although he has bad osteoporosis he tells me, since his 20's and that is way he broke both shoulders when he slipped 5 years ago. He is still having restricted movement in his arms because the tendons were ruptures.
