Dear Rochelle,
As long as the patient feels good after the first prescription it is not wise to repeat or change the remedy. It is also useless to think about what could be the second prescription based on the current symptoms till the patient comes back complaining. Often a patient who is trying homoeopathy for the first time may feel very relieved at the emotional level after taking the potentized medicine. So one cannot be very sure in this case about what is really happening. Regarding this patient she may well require Ignatia - Nat Mur (or in the reverse order) for the trauma she has suffered that may resurface after the present lull. The smell in the vaginal region may not be a new symptom but an after effect of the HRT. The second prescription in this case ought to be W&W, i.e. wait and watch.
