From Toothache to Backache

This to me at least seems like a complicated case and I wanted to put
it out there for feedback.
Female presenting with abssess on a broken tooth. Neuraglic pain
extending to ear. Violent pain at night. Hypericum 200c ameliorates
but seems to be used up rapidly so she repeats regularly. After a few
days with at least one Hypericum 200c per day she developes acute and
severe sciatica from lower back to inner thigh. There is a hx of
lower back injury. Suspecting an aggravation from the hypericum, have
used Kents method of treating an aggravation ie take original picture
and that of the present complaint and find rx which cover both - this
turns out to be Spigella takes this in 200c gives some amelioration
over a 24 period, however the sciatica seems to completely disappears
on arrival of menses. Constitutionally this patient has been
suffering with aggravated per menstrual issues; breast tenderness and
irritability, some depression. She also has a long history with
psorasis.
My question is: Is it right to assume that the sciatica is definitely
an aggravation? Or is a progression of the Permenstrual sx's (good or
bad)as the breast tenderness and irritability were absent with the
sciatica. Would like to have your thoughts on this?

common and how do you deal with them? Incidently coffee, mint were
taken to try to antidote the Hypericum. Also tried a low potency
Hypericum and Arssenicum. Neither worked.

Dear Elizabeth,

To see whether a symptom is the result of the proving
of patient's acute remedy, we should first analyze the
symptoms of her sciatalgia to see if it matches the
hypericum picture of sciatalgia. 'There is a hx of
lower back injury', in my opinion, is not a good
reason to conclude that the patient is under the
proving of Hypericum.

And another point from my own experience. I have seen
that women with many problems associated with menses,
pains, PMS,....are sensitive patients and we should be
careful in choosing the appropriate dose, potency and
repetition. Dose is not clear but I beleive that both
potency and repetition are high for the patient.

And last words... it is really hard to antidote the
effect of a remedy in a hypersensitive patient because
she may enter a new proving!

Hi-
I would consider Hekla Lava, Mercurius, Rhododendron, Staph,
Chamomilla first. Kreosote, Plantago are also good. Sometimes even
which tooth it is matters.
Even constitutionally Chamomilla could be considered -
Irritability, Premenstrual problems etc.If you want to imaginatively
extend Chamomilla's irritability - Psoriasis is a skin manifestation
of it. I have found Chamomilla effective in Psoriasis prescribed on
Mental irritability, menstrual symptoms. Find the aggravating factors
first for the toothache. At this stage probably considering
constitutional factors may not be good. Personally I would seriously
think about Hekla Lava. Later on constitutional prescribing can be
undertaken. Looks like you will have a lot of symptoms to prescribe
on for her chronically.

Dear Elizabeth,

The presenting picture, if in a stabilized form, can be taken
as a definite one for prescription. In the present picture,
the menses amel is a curious symptom. Probe in that angle.

Zinc, Lach , Stan are the three that are amel Menses. I have
a cute little book ''Index of Aggravation (and Amelioration)' by
Edvin A Neatby where so much could be taken. Further at a local
level, backache amel menses is only zinc.Since sciatica is the
last appeared sx, this becomes an important one with its modaities.
Just a thought.

Hi Venkat-
I am a fan of Neatby's as well. The index also appears as part of
their Manual.
The question about the sciatica better by menses is whether this is
sciatica > Menses or Sciatica or Leg pain Before menses. She has
many more premenstrual problems. The whole thing could be a complex
of premenstrual symptoms.In which case it is a general aggravation
before menses symptom.

Your thought process has been very good. Zincum looks good. Esp. if
we think about all the suppressions for psoriasis that could have
taken place.
But I still think we would need more specific modalities for the
pain. If toothache alt with sciatica it could be Ignatia.This might
be also Staph which has Before menses symptoms and of course dental
problems as she has had.

Hi Yekkirala-

Your analysis is good. What is EH though? Mean while did the case say
any thing about suppressed menses. Amel by menses does not mean there
is suppressed menses, does it? Between menses neuralgic pains is only
to be used if there is a clear cut concomitant relationship between
menses and the pains occurrence. That is what even your quotes say -
" the patient may
half way between the periods. Which may not be the case here. I still
think we need more descriptive modalities - for the sciatica, for the
psoriasis and even for the toothache ( abscess). We do not even have
a description of the nature of the pain. "neuralgic" pain is just an
explanation by the doctor. probably in conditions of pain it is
better to have a clear description of the sensation, The description
itself might aid diagnosis of the remedy - right?.

In the case under discussion, her sciatica ceased on appearance of menses.
So the link between the two can be either way. If the menses had not
appeared her sciatica would have continued. So we may take the remedies that
have this kind of interlink and we are not going to give any remedy without
checking it back in the MM anyway. Asa you said we need more case details
and modalities which I had alraedy asked in my previous post.

This lady is amel by a natural discharge. This is the
point unless the menses is erratic. This becomes a
condition which points out the miasm (psora) and
fortunately it is a rubric not large enough to
disallow elimination. But this is only one of the legs
of the chair and to make it stand the other two
reliable legs are to be found and fixed.
