Rubric help -- dissolved

I have a client who has said a number of times on a number of occasions that when she is getting a headache (usually migraines -- but she has no details to give because of this other thing) she feels as if she is dissolving, and she can't think, can't do anything, needs someone else to tell her to go to the fridge, get out the ginger ale, pour herself some, etc.... -- really simple things.
I questioned her really carefully -- she doesn't feel as if she's separated in any way, and there's no sense of duality, no sense of any other power... just that she's not there anymore.
Obviously confusion of mind, perhaps confusion of identity, but is there any more specific rubric? This is central to the case.
Nat-m was very helpful for several months, but I haven't been able to find the next useful remedy, and Nat-m stopped working in any potency. The centrality of headaches makes me wonder about Kali-br or Thuja, but there's nothing that really catches the imagination among the rubrics or core symptoms for those remedies, outside the headaches, and the headaches don't really fit, as far as I can tell with so little to go on.
One might think of "absence," as well, but I understand that rubric as involving total absence, as in petit mal seizures, and she hasn't had anything identifiable along those lines -- just migraines and muscle tension headaches.
Obviously it's some sort of dissociation, perhaps even to the extent of catatonia, but again, not quite -- with outside direction she's perfectly capable of doing whatever she's told -- she just can't think independently to do what she wants. The case is long and complex, and I'm just wondering if anyone has a rubric thought.
