Minutus

A patient had a psychotic episode during which she had delusions, amongst them delusions that if she looked at someone it would kill them, so she was scared to look at anyone.
In addition, she had black-outs, would find herself in a different place and realize she'd lost several hours and had no idea where she had been.
She has come for a different reason, but I think that the episode, which was several years ago, still has bearing on the case. Since the attack she's been on psychiatric drugs.
Any ideas?

Thanks for presenting the different perspective! I think you're right, that there's a certain amount of superstition that came out during the psychotic episode. According to what she told me, she only had the delusion during the psychotic episode, not before or since, but I think she is superstitious underneath it all. She said that she did not want to look at people as she might kill them, but on the whole she seems quite gentle. She also definitely believes she has psychic powers and has had clear instances of clairvoyance.

subject header Tarentula has something along these lines. Rubric =
Mind, looking at others and materia medica would come in the form of
'when there are no observers there is no hysteria' so it is a cunning

rubrics - there is 'fierce' and 'wild' and you could cross refer to
Eyes, wild but when the look is one that could kill the whole facial

Hi Joy,
The background is that during a psychotic episode this patient was scared to look at people as she felt if she looked at them it would kill them. Caro directed me to think in terms of superstition or belief that she has psychic powers as underlying the symptom. So far, in the repping I've done, Phosphorous shows up heavily through mental and physical symptoms (sorry can't post), although Tarentula is present. She's especially scared of spiders - although that seems to trite. How is Tarentula in the superstition/belief in her psychic powers area? I would expect there to be more nervous energy in a Tarentula patient. She was quite still, but then she's on medication. She has said that when she comes off the medication she feels an over-excitement of the senses.

If the expression truly comes through the psychotic episode and isn't
related to any medications then the emphasis should also shift to the
element of 'wanting to kill', albeit with the eyes and not with
anything else. It would obviously be important to explore the state
when off the medication and find out the extent of the 'over excitement
of the senses'. Most of the spiders can be both over excited and
extremely still (part of the watching for prey stance). Tarentula
certainly has 'threatens to kill' and the look with eyes says that.

Superstition is a fascinating state and it would be worth getting her
meaning of it because although we often take it to be an unusual fear
of the unknown and a link to the supernatural, it can also has other
meanings such as to stand over and be a witness to and if you put the 2
together there could be the watching and the killing - the waiting to
kill which is more manipulative than that of Phosphorus which
definitely has the rage to kill (not a well known sx but it is there)
but it isn't necessarily a deceitful one. I don't have a rep with me at
the moment but I would be surprised if some of the spiders aren't in
the superstitious rubric.

Perhaps delusions he/she will murder would be appropriate - kali brom,
stramonium types? Can you drop any other hints about the case? Best

If she is on antipsychotic medication probably this is not
superstition- nobody would get prescriptions for being
superstitious... If we take this as a delusion - that she is capable
of killing others with her stare - she is either manic or
schizophrenic( paranoidal?) - probably you should get more about her
psychotic episodes - what causes them etc. how this whole thing
started etc.
since she also has belief of psychic powers and clairvoyance- then
you probably need to enquire about depressive episodes because mania
is psychodynamically a compensation to depression...

If it is confirmed that she indeed had a psychotic episode then apart
from than the mental symptom the physicals need to be covered...
because in mental disorders the mental symptoms become common
symptoms and the physical peculiarities if any would be guiding. If
it is not a delusion but superstition ( which seems unlikely in this
case) then this symptom is valuable more...See if you can develop a
picture of the patient before she was put on medication - what were
the symptoms that were present that the patient had to go for
treatment? Is there any one else in the family who can corroborate
the history?

How did she adjust to this? how does she react to this? Modalities?
seasonal? How often does this happen? WHat happens in the interim?
Sleep? Menses? Does she think this is correct? Is she reacting like
this only towards "bad" people or with everyone?

You said she had other delusions as well - are these also related to
the same theme or are there other kinds? That might clarify the
picture further... black outs fugue states etc. might also be
significant... get details of these episodes - time line - how long
do these occur etc.
Whatever medicine you select should of necessity cover the current
complaints she has come to you for...
