headache/sinus case

I've been treating a now 45 year old woman since November/02. I have
not yet found the similimum. Please help! Here is the complete case.
Any thoughts will be appreciated. Thanks!

Patient's Medical History:
Anemia, Influenza, Measles, Mononucleosis, Osteopina, Strep throat,
Sinusitis

Family Medical history:
Anemia, Arthritis, Asthma, Epilepsy, Heard disease, Osteoporosis, Thyroid

Injuries/Surgery
Water on knee - age 8
Back Injury - age 17

Physical appearance and my observations
- very slim, medium height with dark hair
- intellectual - cannot express emotions
- seems to speak through clenched teeth
- has a Nat-m-ish quality to her

Main concern: Headaches - started 13 years ago
- had beer in a restaurant and suddenly a sharp pain like an ax
across head
- lasted for days
- took "feverfew" and tried acupuncture - nothing helped

Since March/02 the headaches are different
- starts as R sinus pain - approximately once a month
- monthly headaches, starts on R eye and moves upwards settling on top R
- can be on left as well
- headaches last 3 days, they can start 4- 10 days before start of period
- dull pain with heaviness
- usually wake up in a.m. with a headache "it can wake me up if severe"
rest
Weakness and achyness in knee

No pregnancies, never married. Last relationship was 10 years ago. "I
am very picky".

Personality
- I used to be a teacher, but left teaching years ago. I liked
students who wanted to learn but didn't have the creativity to motivate
the ones that did not
- I like things in a neat orderly fashion
- don't like clutter or messy things
- feel calmer in neat and tidy surroundings
- enjoy routine
- I never get excited
- always planning my day - have lists
- love time alone - crave one day a week to read, cook or bike
- very rarely get angry - keep anger in
- get sad more than angry
- feel guilty after becoming angry - they won't like me any more
- if physical security is threatened can lash out - happened a few
times as a kid
- get embarrassed easily
- get bored easily
- cannot cry in front of people, it shows weakness and no control
- usually up person, but can get depressed especially in July/Aug
- get anxious when people are late or when I have to wait for
something to start
- get anxious about having things in a certain way - orderly etc.
- if I think I can't control something can get me anxious
- must move around - strong internal need to move, exercise
- don't like confrontation, will avoid it at any cost
- when people give me the cold shoulder I take it personally
- I blame myself when things don't work out
- When growing up it was important for people to like me, I would be
tortured if they didn't. It's not so important now
- I can get envious over close relationships
- biggest worry "if people like me"

Fears/phobias
- drowning in salt water - mom made me gargle with salt water when I
had strep throat
- strange sounds at night - feel as though someone got in
- fear of painful death
- fear of poverty at old age, and no one to take care of me

Childhood
- pretty happy
- we moved around a lot
- I remember: (she remembers only the physical accidents)
o falling off bicycle and chipping front tooth
o climbing a tree falling out
o brother's hand getting caught between car door
o falling and hurting knee - leg so swollen couldn't sit in the back seat

- remember waking up and thinking I was all alone in the house at age
2 - when brother was born
- difficult relationship with mother- she expects me to be married
(why, why?)
- mother is very critical of me
- difficult relationship with brother - he's always right, it's like
walking on eggshells
- I feel as though I am the central support of the family
My brother married had 3 kids and he's a deadbeat father to those children
He's always away from the family.

Remedies given since initial consultation in November/02. The ones that
helped were repeated without any results.

Ars. 30 - made her agitated, impatient and nervous

Kali-muir 1M - calmed her down - headaches were shorter but more severe

Sepia 200 - no headache for over a month then back to normal, Repeated
Rx with no difference

Carc LM 1 - headaches less frequent and lasting shorter, but nose more
and more dry and bleeding

Dear Tamara, I was beginning to think about Nux vomica and Staphysagria when
reading your case but I think there are far too many questions that need
answering to offer anything really positive.

You say she is intellectual and cannot express her feelings - why is this

Does she physically speak through clenched teeth or is this your
interpretation

The way the headaches began is interesting - what else was going on in her
life at that time

What >>> the headaches

You say she has internal restlessness - what causes this

In what way and why is she 'picky' about her relationships

She says she never gets excited - what does this mean

You write that 'she rarely gets angry - keeps anger in' - which is it? If
she keeps her anger in then she is getting angry and you need to know more
about this

How and why did she lash out as a kid when her security was threatened -
this is something that is still part of the case and so you need to know
more

Why does she get depressed in July/August

What sort of things embarrass her and how does she react.

How does she react if she has to face a confrontation

Why does she take things personally and why does she blame herself for
things not turning out right

Why is so much order needed in her life.

What work does she do now.

I am sure if you can get some answers to these sort of questions you will
open the case up enough.

One question that occurs to me right off, with the frequency of once a month is to wonder how this relates to her menses, if there is any steady relationship, and if there are any other concomitants having to do with menses....
