[H] Request Advice : Case of darkened smelly crotch

Dear List Members,

I have been treating a patient for the last three months, the chief symptoms of whom are as follows :-

1. The area of the skin in the fold of the thigh and the scrotum has darkened and is dark brown/purplish/reddish in colour. Initially the darkening took place only in the front but has now progressed all around the fold of the thigh ....right up to the anal region.

2. The smell of the skin in this area is very very offensive....something like decaying flesh/ something dead. In fact a finger rubbed in the area just cannot be smelt......it is that offensive.

3. After a bath/ shower the skin gets tender and can be rubbed with a finger and .....it peels off easily uncovering a shining inelastic skin underneath.

4. Perspiration seems to aggravate.....and the patient gets a slimy feeling in the area between the thighs and the scrotum.

The treatment was PSORINUM 200 ..given as four split doses in water over two days.
The area seemed to dry up and the smell slightly less. After waiting for a period of nearly one and a half .....the patient was given PSORINUM 1M ....given as two split doses in water over two days. There was no reaction..It has been nearly a month and a half since the last dose.

The problem remains nearly the same and there has been no improvement.

Apparently Psor. was only very slightly effective in palliating - so not
at all even close to a partial simillimum.

Have you taken a full case, as opposed to only focussing on the local
lesions ?

About 11 remedies in the rubric below are also known for fetid odor.

Could it be an erysipelatous infection, with some gangrene???? If so, Kali-p is one possibly indicated remedy (one thinks also, of course, with the smell coming from the skin itself, of Pyrogen or Asaf, or Secale, or Anthracinum, or even Ars, or Lach (which is in SKIN-eruptions fetid along with SKIN-discoloration purple around eruptions). There are other remedies for putrid perspiration.... but your description does sound necrotic -- syphilitic.

The following description of erysipelas is from Yasgur, and may be of some interest in finding rubrics that fit the situation:

(rose, St. Anthony's Fire, erysipelas faciei, ignis sacer) superficial cellulitis (fever and local redness), a contagious disease caused by group A beta-hemolytic streptococci. The lesion is usually well delineated, shiny red, edematous and tender. Vesicles and bullae often develop. Erysipelas often attacks a wound; if not entirely cleared up it can become a source (nidus) of continued periodic infections, often recurring year after year. High fever, chills, and malaise are common. It frequently occurs without obvious injury. If unchecked the infection may spread to deeper tissue producing necrosis, gangrene, pneumonia, nephritis, and meningitis. E. genarum would refer to the cheek area.

This connection was elucidated by Tuiller in 1630. It is called 'St. Anthony's fire' because persons so stricken were cared for by the Order of St. Anthony in their hospices. So these terms are a confused and jumbled lot. 'St. Anthony's fire' in homeopathic literature refers to erysipelas and sometimes to gangrene. Yet be aware that 'St. Anthony's fire' may also refer to ergotism.

The other symptoms, as usual will give clues to which remedy works. I for one will be interested to learn what happens.

There has been no fever....that was ever connected with this symptom.......Anyway, I shall soon be retaking the case and writing about it on the list...
