just to be provocative, there are always those patients who won't or can't open up in personal interview but can express their feelings much more freely in writing, and present their symptoms more clearly.

Probably many!
IMO the ideal is to have face-to-face, but make it clear to the patient
that, if they later remember anything they wish they had told, they can
write you a letter or e-mail to get it covered. Sometimes useful things
come out that way!

I agree that people open up more when filling out questionnaires at home,
but it's quite productive if they then bring them to the clinic for
discussion.
