I have one of those difficult cases which is plodding along but not getting there. She phoned me today to tell me how she is doing.

The latest I have got out of this 55 yr old woman with a fatigue syndrome caused initially from RSI is that the pain is better when the adrenalin is running. She has been buying a new car and has a big anxiety if it will be a good purchase.

"Have I done the right thing"?"I do something and then I doubt myself and exaggerate what may go wrong."

While she was going through all this her pain was not as evident so maybe it is a case of occupation amel rather than excitement amel. Apparently the same happened when she bought a new flat. Once she got the car the pains (whole body aching when she does too much )have been worse again .

The other thing she gets is a vertigo, sickness from riding on a train even for a couple of hours afterwards. She feels as if she is not quite walking in a straight line.

She is obsessed with her bowels but at the moment the remedy has improved them so that they are not as sluggish and she is getting an urging which is making her perform a lot faster with a looser stool. There is also fresh blood when she wipes herself from haemorrhoids.

More new evidence emerged of a possible fear of being enclosed and she is terrified to watch programmes on caving. She felt enclosed in a marquee recently because of the disorientation she felt from the noise around her.

I have discussed this case before and she is on the occasional Sanic 1M in a WP. The last time she took a dose was about 2 weeks ago which made her bowels sluggish for the first day ad she gauges how the rx is working on the state of her bowels.

If anyone has any comments on the above I would be interested to hear them.

Have you explored the reason why she can't watch caving - is it that
something is lurking, dark, usually enclosed but not always as some caves go
on for ever. Perhaps you need to get a bit nearer to the level and cause of
the anxiety that prevails.

Is the anxiety anything like Arg nit - anxiety when something is expected of
them, when they are expected to perform well but expect everything to go
wrong. Lyc has this also as do many other rx - both these have haemorrhoids.
Or is it more like a Gelsemium situation? I am only mentioning these rx as
there are hints of them in what you write.

What is the 'obsession with the bowels' all about, can you explain more.

She looked on the bowel problem (which I didn't really think it was) as a marker to whether the Rx is working. Now the marker is the pain and the vertigo.

I did try to explore the caving ad all I got was a "the thought of being enclosed". OK so the next step is to find what "being enclosed" means to her.!!

I would assume at least, with the "obsession about bowels," that "anxiety about her health" would be applicable and important. FWIW

But, I would ask her why she has this concern with her bowels, when and
where it stems from. If it's longstanding or traceable to a particular
time, it could be significant, but It could simply be a learned concern
(which could either change the rubric you'd choose, or could make it
irrelevant as a symptom). Or it could be something that she has found
empirically to be helpful for her (in which case you'd want to rep the
effects of constipation or whatever, rather than the concern with it).

I think I would have to be very diplomatic as I think she would be most offended if I dared to suggest to her that she had an obsession with her bowels!! She doesn't feel she can get on with her day until she has emptied her bowels to her satisfaction. She was a radiologist and talks in terms of poor peristalsis of the bowel and the lack of urging + the feeling of an insufficient movement. However as I said this is not what she is going on about at the moment.
