Cystitis (sent via the website!!)

I have a patient who has cystitis pains. This is a complicated case
but basically in November she had an operation called TVT which
stopped her stress incontinence. However it did not stop the urging
for which she has recently begun to take Detrusotol an allopathic
drug. This works for her. Before the op she was on an allopathic
medication which virtually stopped the stress and the urging but she
had the op because she did not wish to continue the medcation.
I saw her in April 3 weeks ago when she described :-
"It's smarting and there is pain, very low down inside < pressure
or bending. Pain starts gradually – must sit down. The worst thing
is to stand still, next is walking and it is best sat down or lying.
It varies sometimes it is < after urinating < during urination.
Sometimes hard to start urination."
If she has a meal and has a glass of wine after a couple of sips the
pain starts off. It is < initially and then she waits for it to pass
off so she can drink the rest of the wine. Time makes it better.
The pain lasts about 7 minutes. She wonders if this new symptom is a
side effect of the operation.
"I started taking medication for the urging and leak about 2 weeks
ago as it was too difficult to manage."
After she took the Staph 1M last time I saw her the pain went away
for 4½ weeks although the urging and leak continued. There is no
stress incontinence now as the operation appears to have
successfully dealt with this.
I repeated the Staph 1M and after an initial 2 hour agg was fine for
3 days. I then had an email telling me that she had an acute pain
with urge to pass urine and when she went every 40 mins or so she
felt saw and smarting inside. I told her to take another Staph 1M.
after which she was fine until an email tonight which she wrote:-
"I am sorry to say that the cystitus pains/discomfort have returned.
On and off over the last few days.It just comes on slowly is a
strong pain for say two hours and then fades again. The 'worst'
aspect varies, sometimes before urinating, sometimes during and
sometimes on completion
I am still taking the Detrusotol 4mg
I have just begun Staph 30 and will take one each half hour over
the next two hours.
If there is no improvement then may I take one of the others?
I have a 200 and one 10M .
I just hope that the problem is nothing to do with the op, or the
tape they used or something worse ?prolapse?"
I have sent her an email telling her to stop the 30C and to phone me
in the morning as it is late here.
Anyone any suggestions to my next step. She has done well on Staph.
emotionally which we started in October with 30C.
My thoughts are:-
1) Give her Staph 1M in a WP daily until improvement
2) Give 1 dose of Staph 10M
3) Change the Rx to possibly Causticum which was the first Rx she
ever had from my last July which did wonders for her confidence but
nothing for the Stress Urination.
All suggestions much appreciated- preferably before she phones
tomorrow morning!!!!:-)
