I have a patient who is 7 months pregnant and has thrush. It presents as bland, white/cream, thick, itchy and has a yeasty smell.

I only started to treat this patient during pregnancy and she has done well on Sepia 200 and going up to 1M when there was a return to tiredness which the 200 did not cope with. This still didn't work and at the next consultation she told me she was tired, weepy, breathless and had palpitations after eating. Thrush had started. There was also constipation with haemorrhoids. The weeping was the main think that needed dealing with and Puls 200 , even repeated failed to make an impression. The constipation and haemorrhoids are better more due I think to the addition of Flax see to her diet. However the headaches she was getting went away. Today she appeared very chilled out but we still have the weepy at trivial things, the thrush and what emerged was a great fear of the birth (her second) because of being out of control with pain as last time everything went wrong and she doesn't trust the nurses or doctors and a fear of being ill afterwards , like last time and of not coping with the baby. She thinks it stems from anger from the last birth. She is still showing Puls signs of vulnerability with a feeling of being unloved by her partner (also a patient). She is craving ice to crunch. I would be interested to know what others think I should give now. I want to stop the weeping which is her first concern, next the thrush and also the fear. I have given her some Bach Fl. Rx of Mimulus and White chestnut although her sleep is fine.

Lac caninum is in that category. Has the thrush and the fear of loss of control of situations, which is presenting complaint (next child delivery which went so badly last time). Feels like husband doesnt like her and Weepy. List below a cured case of Lac-c that desired ice. So that's one idea for the mill.....

DIFFERENTIAL COMPARISON WITH SIMILAR REMEDIES.
Lac Can. may easily be confused with Lachesis, Argentum Nit., or Phosphorus.
Like Lachesis Lac Can has an affinity for the throat and female genitals.
It prefers the left ovary. It also has a marked aggravation around menstruation. Like Lachesis there is fear of snakes, and aversion to pressure which aggravates their conditions.
Lachesis is also often highly nervous and intense, having feelings of floating and delusions or fears about snakes. The two remedies may be complementary in some cases.
Lac Can shares with Arsenicum Album a fear of losing control, and a threatened feeling. The difference is that the enemy of Lac Can lies within, whereas Arsenicum's existential insecurity is over outer threats.
Lac Can, like Arsenicum has fear of those things that signify loss of control. Thus there is fear of incurable disease, fear of death, fear of insanity, fear of falling and of fainting. Both these remedies desire company.
Argentum Nit. is the closest remedy to Lac Can when the mental symptoms are compared. They are both in the rubric, 'Delusions she is despised' and 'Delusions of snakes'. They have similar fears. Both have fears of falling, of fainting, of insanity and of disease. They may be difficult to tell apart. Their physical symptoms may serve to differentiate.
Phosphorus with its basically pleasant disposition, nervousness, heightened imagination, and fear of disease may also be confused with Lac.Can.

SUMMARY OF CONFIRMATORY SYMPTOMS.
Guilt or Self Disgust
Fear of disease particularly cancer.
Fear of fainting or collapse.
Fear of snakes.
Fear of insanity.
Great nervousness and anxiety, with feelings of losing control.
Sensitivity of the nerves.
Sexual problems - guilt or oversensitivity.
PMS with sore breasts, worse from slightest touch or jar.
Scanty breast milk (Lac Defloratum).
Throat problems. Glistening white or red patches.
Pains alternate sides.
Worse around menses, touch, jar, cold air.
Better cold drinks, open air.

She has seen a Neurologist, Osteopath, had complete and thourough evaluations, with MRI's, EMG's, CAT's etc. etc. Many Doctors, many meds - no help.
Sacro illiac joint goes out. Lies on couch daily, couldn't walk, couldn't reach. Pain goes R-L-R-L every few weeks. Shoulder worse extending her arm. Goes into spasm down right arm and muscle stands up. Trapezeus huge and like ropes. Left buttocks pain all the way down to feet. "Rock like" feeling, sudden and intense (also chronic), felt like screaming. better ham string stretches sometimes. GI: Desires: Salt, chips, sweets, beef, fruit oranges, ice, soda. Intentionally lost 24 lbs in last month. Aversions: Spicey, pepper. Thirsty for icy cold (dry mouth and sinus). Normal digestion.
Menses: Regular, no cramps.
Temp: Chilly, hands and feet, better bio feedback, likes hot sun but gets rash in it. BP: 146/98, goes up with anger.
Meds: Capotin Maxide, Feldine, Prednisone, Ceclor, Angmentin, birth control pills, Valium, Flexoril, Naperson, Sudafed and contact for sinus.
Sex: down in last six months. "Can't throw legs back, it hurts" SI - joints. Intense orgasm, whole body, tension, sexual energy used to be high. Doesn't want a child because of freedom and health.
R.t: R. to L. hand. Numbness in 3-4-5 fingers, and to Left arm. Left Heel pain, couldn't walk went into whole foot. Heels get so sensitive she can't put feet down and walk. Worse one month ago, better hot bath.

Taking cue from Andy's rubric on ice desire and eliminating with weeping ,
trifles
Natm, sil and calc.
Pregnancy during: Nat m.
Now I see the past hurts resurfacing which may need nat m.
Rest, the thrush has to be thrashed.

You've had some good suggestions from Venkat and Andy.
I can confirm that salt rather than sugar can be the basis for some
"yeast/thrush" problems and since Nat-m is complementary to Sepia which is a
Rx she's responded seemingly well to, it's worth a thought!

However, it could be that the appearance of this corrosive leucorrhoea is
part of her healing process and the Sepia is still doing good work.
Certainly Med and Lac-c come to mind and Andy has presented cogent arguments
for considering either of these.

Still, I'd be inclined, since she seems to be getting some relief from diet
already, to pursue that and give her Morgan pure to support the dietary /
bowel flora approach.
It is "complementary" to both Sepia and Med and, in my clinical experience
to all the Lacs also.

Genitalia: Pruritis vulvae and vagina +++.....leucorrhoea corrosive,
offensive.....bartholinitis....

Mentals include:Tense; active, weepy; depressed; irritable; introspective;
anxious and apprehensive about state of health (? impending birth?)

Haemorrhoids and constipation are part of the Rx picture; so is
breathlessness.

Morg-p is generally a hot rather than chilly Rx but the desire for ice makes
me consider pica which can be related to iron deficiency so she could be
lacking vital heat due to anaemia. Have her iron levels been checked lately?
If she's Sepia she probably has been "off" red meat?

Can you let us know what her relationship is with her mother as it would be
important to look at that if you are going to consider Lac-c.

Hi Patricia, Andy and Venkat

One of my problems is that the thrush is bland - not corrosive and I am sure I wrote this. I did think of Med but really feel I wouldn't wish to give it in pregnancy when I am not sure. Nat Mur is a possibility. She tends to be angry with her Mother as she says that she interferes.
I have just had a thought about Opium because maybe she has pictures of what happened in the last birth where the gas and air cylinder exploded during a home birth, she was transferred to hospital was put on Pethidine which didn't work, had a Venthouse delivery, lost a lot of blood and had a transfusion. She also got mastitis.

I have to say that the ice also made me think of pica because it is crunching ice she desires not iced water!!
