Concise casetaking checklist

Hi

At my current stage of training I am now taking patients under the
supervision of an experienced homeopath! - both an exciting and scary
experience!

I was wondering if anyone has come across a good concise 'casetaking
checklist' / 'memoryjog list' that they have found useful. I am often wary
that I may not have requested all the necessary info - although I am aware
that in most cases the patients gives what info is needed if allowed time
and space!

Any ideas?

Hi,
Each one has to develop his own style of case taking and it is better we
prepare
our own case taking form to collect the information prior to the
consultation/
interview.
The ambitious student should master the art of interrogation by a close
study of books by Pierre Schmidt (The Art of Interrogation), Rajan Sankaran
(Heart of Case Taking) and

the series on case taking articles by Dr.Will Taylor, M.D at

Then it isn't spontaneous and you don't have a clue what the person thinks
is the most important - it shifts the interview toward what they have
answered on the form
Not something I would ever use, personally
