Hi Colleagues

Patient says that his head feels icy cold (especially his forehead) and the
more his body is hot the colder his head feels and hurts.
If he has his head under a hot shower the pain goes.

If he has his body cold, his head feels warmer and more comfortable.

Any ideas pls?

I'd look at :

Chill - single parts, (as if chill running through one or another part).
or
Chill - single parts, internal.
in BB Repertory pages 1024, 1025 and 1027
or
External Chill with Internal Heat. in the compound fevers chapter P. 1099.

Even if your case is not strictly a "fever" case, be aware that *fever* in
Boenninghausen's context was a general term for any abnormality in
Circulation, so the rubrics may still apply.

Ice, in any shape or form, always makes me think of Agaricus, Camphora,
Arsenicum; Veratrum; Valerian; Gelsemium; Phosphorus; Indigo; Hellebore;
Cistus; Carbo veg and Calcarea Carbonica.

Remember that ice always has a relationship with heat because ice can burn,
so nearly all these remedies, and others, will have characteristic symptoms
regarding ice and warmth. Carbo veg, for example, has the opposite of what
you ask about - head hot with cold limbs; Indigo has the sensation as if the
head were frozen but sensations of heat elsewhere in the body; Valerian has
a great sense of coldness in the head and alternating symptoms, so this
might be relevant; Camphora has icy coldness which alternates with burning
and very sensitive to both cold and warmth.

'Headache on getting head cold' might be a useful rubric apart from other
obvious 'icy' ones.
