Hi Anna,
Regarding brain hemorrhage there are a few things we have to keep in mind. First of all Causticum is rarely indicated in recent cases of paralysis after brain hemorrhage. The paralysis of Causticum is slow developing. Sometimes if after treatment residual paralysis remains Causticum may be indicated but as a rule causticum does not have much role to play in post hemorrhagic paralysis. The medicines in such cases depends a lot on when you are seeing the patient. If immediate some useful medicines are Art-v, Apis, Arnica, opium and bell. Bothrops may be needed after 24 or 48 hours when the case has stabilized a little. But again other remedies may be needed if the paralysis is on the left side Lach is more useful than any other remedy. Right sided paralysis may call for crot-c, Both., crot-h, phos. op etc.
When the patient improves but does not get complete recovery constitutional remedy may be given. Here remedies like Caust., Plb., Phos etc. come in handy.
Some things to look for are first the CT scan to see what is causing more trouble if it is the size of the hemorrhage then Arnica, Op, bell lach etc. may be better if edema is causing more trouble then Apis, may be better option. The side is very very important. Not the side of the hemorrhage as some people claim but the side of paralysis(which would be opposite the hemorrhage). Again are there convulsions. If there are convulsions which side are they on are they on the paralyzed side or the opposite side. How deep is the coma. Opium and lach have deep coma whereas in bothrops there may be no coma but the patient is unable to speak.
Look for any involuntary movements. I remember one case where the unconscious patient kept taking his hands to his face as if brushing something away. He had been under medical treatment for more than a month and was not gaining consciousness. I had also tried some remedy though I don't remember what before I noticed this symptom. I asked a few questions and gave hyoscymous 50M. In 24 hours he was awake. In a few days he was moving his paralyzed hand and foot.
The above is just some general tips basically if the patient is only paralyzed but in his senses then treat the case like any other case. If unconscious keep looking till you find a symptom.
Best regards
Elham
PS: Some useful rubrics are cerebral hemorrhage, hemiplagia, apoplexy etc.
PPS: By the way by giving bothrops and arnica we are not treating the cause. If for example the paralysis had come after indignation and you gave staph. That may be considered as giving on cause.

Re: BRAIN HAEMORRHAGE
Hello Elham,
Thank you for the great reply!
Pt is already (almost) 3 years after brain
hemorrhage-and it's her second one (first was 15 years
before after which she had facial paralysis-left
side). After 2nd-she has additionaly lateral recti eye
muscle paralysis -left -and also right side numbness.
Causticum seems to be indicated in all of the above-
besides it's her constitutional, I think. (there are
no convulsions or unconsciousness)
Arnica and bothrops both cause hemorrhaging in healthy
people-and the here the hemorrhage caused all the
symptoms-wouldn't bothrops (right side affected) be
indicated here basing the prescrption on the
causative factor?
Or maybe I totaly miss it.
Do you (or someone here) have (or had) a pt after
brain hemorrhage? These people really need help.
