What is the purpose of the questionnaire?
Without being clear on what you want from it, it is impossible top advise.
Most questionnaires I have seen that were for the purpose of finding
a remedy were failures. They asked too many closed questions ("do you
like xxxxx?")

Julian: I was reading your review on the Bamb-a proving by B. Schuster. There was another review that was very critical of the book. Do you know of any good cases of Bamb-a?

i have had a small experience with bamba. woman with structural problems--knees, back, neck
lack of support in her life as a child--still feeling unsupported, fearful and anxious.
bamba in increasing LM's over several months. improvement in some complaints. burning in
neck when under stress ameliorated. more work required on case.
i know this isnt very detailed but just an idea.

No I don't. But I'm not that up on current literature.
Maybe someone else on the list would know.

Thanks Tanya, it comes up so often in the rubrics but I have only heard of one case and lack of support was also striking - one thing I was looking for specifically - was there any hysteria?

DR. Gardner has one of the best questionnaires I've seen.
