Dear Fellows,
Please help me to find out correct remedy and/or most suitable course of treatment for this case, forwarded to me by my homeopath friend, practicing in London:

"He is in his early thirties, Belgian, a huge man,
well over 6 foot, and big built, but very gentle. He
has just gone to work in Zaire after the break up of
his relationship. I was treating both of them for
various things, including his impotence. The main
symptom he keeps bringing back to me is his chronic
sinus problem - bloody discharge or green/yellow
with blood streaks, that causes bloody crusts humid weather, < dry weather, < cold,
damp. He works in the metal construction industry,
and there is a lot of metal dust at times - or has
been in the past, I'm not sure what his present job
is. The sinus problem started in India, when he
first started to smoke cigarettes, had a lot of
problems, lost his money and passport. It came on 2
weeks after a strange experience while meditating,
concomitant with headache. A few months later he was
ski-ing in France when he was
completely overcome with sinusitis and head pain,
took antibiotics and nasal spray, thinks it was
brought on by exposure to cold.
Frequent sneezing - can't stop.
The pain is above his nose < pressure on brow.
Pressure above eyes.
Every morning it is completely blocked. Snoring is a
problem, sleeps with mouth open.

(I gave him Phosphorus 6c for this, and it helped
for a while, but then stopped helping, so I raised
it to 30c - it helped but then stopped working
again).

The impotence:

Can be very sensitive during sex, over whole body,
aroused, but no erection. Can ejaculate without
erection. Or erections don't last long enough, even
when aroused.
When I first started seeing him, he had recently
stopped smoking and associated the impotence with
that, but the problem seems to go back a long way.
Major trauma with last girlfriend, when the string
on her IUD cut into his foreskin - both she and he
were bleeding profusely, both very shocked. Now the
foreskin is not attached. Impotence worse after
that, not surprisingly.

I gave Nux vomica to start with, and this really
helped, gave good erections. But then came a
problem with anticipation - if he went away, he
really missed his girlfriend, and when he came back
was so excited to see her again, but could not get
it up, and sometimes couldn't ejaculate.

Then I gave Caladium (problem associated with
smoking, was a heavy smoker), and this had a big
effect, but it didn't last.

He drinks a lot, and eats a lot. Is thirsty. Has
little self control. Is contradictory, stubborn (his
girlfriend said). His brother has been accused of
paedophilia and the court case is coming up soon - I
feel there is a lot of shame attached to this for my
patient, as his parents are very upset)

He was very depressed after the break-up. Couldn't
sleep apart from between 12-2.30am, then awake on
and off all night, very restless. Strangely, at
that time, he said he was "completely back to
normal" sexually, though he didn't have a sexual
partner.

Now, in Kinshasa, he is not having morning erections
at all. No partner, he says, so he can't "check it
in action". Still has sleep problems. Not
interested in food, can go without.

Has periodic allergy rash on his arms, twice a year
< summer. Cries easily, is emotional. Loves company
(except when he was depressed, when he went into his
shell)

I sent him Graphites, which was well indicated, but
it hasn't done much so far.

Other remedies I have given him - Tub IM (for miasm
- loves to travel, restless), Pulsatilla 6c, Natrum
mur 1M (depression), Arg nit (anticipation). I feel
somewhat surprised that he keeps coming back to me,
as I haven't had a great deal of success - or at
least, not long-lasting. But he likes to have
someone to talk to, I think.

I have to send him a remedy via a friend who is
going to Zaire on the 2nd. I am going to recommend
that the friend buys Thuja 200c, to be taken as a
split dose over one day. This covers the sinus
symptoms, and history of vaccines, and is also
miasmatic, so it may address the problem that is
stopping the other remedies working. But it doesn't
cover the impotence."
